- Wage Determinations Notice to Offerors and Clauses Statement of Work NOTICE TO OFFERORS 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 - Streamlines Procedures for Evaluation and Solicitation for Commercial Items supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the solicitation number is W912L3-14-T-0021. 2. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-74 effective 30 May 2014 and Defense Federal Acquisition Regulation Supplement (DFARS) DPN 20140528 effective (28 May 2014) edition and Army Federal Acquisition Regulation Current to Revision 1. The complete text of any of the clauses and provisions may be accessed in full text at http://farsite.hill.af.mil. 3. This Request for Quotation (RFQ) is being issued to establish a fixed price contract to provide Emergency Management Coordination services at Wyoming Military Department, Cheyenne, WY 82009. The services shall be performed in accordance with the Statement of Work (SOW) provided at Attachment 1. 4. Any award resulting from this solicitation will be issued on a Standard Form (SF) 1449. 5. This RFQ will be awarded on a service disabled veteran owned set aside. The North American Industry Classification System (NAICS) code for this requirement is 541611 and the size standard is $14,000,000.00. 6. Period of Performance shall be on or about: Base Year: 1 September 2014 through 30 August 2015 Option Year 1: 1 September 2015 through 30 august 2016 7. This requirement is subject to the Service Contract Act. Wage Determination No. 05-2587 Rev. 12 dated 6/25/2013 is provided as Attachment 2. Pursuant to FAR 52.222-42, Statement of Equivalent Rates for Federal Hires, the following data is provided for informational purposes only and is not a Wage Determination. The General Schedule (GS) equivalent to this position is as follows: Employee Class Monetary Wage - Fringe Benefits Emergency Management Coordinator $29.84 8. INSURANCE: Any required insurance certifications must be submitted to the Contracting Officer within ten (10) days after the date of contract award as referenced in FAR 52.228-5. 9. NOTE: To receive an award resulting from this solicitation, offerors MUST be registered in the System Management Award (SAM) database IAW FAR 52.212-1(k) and DFARS 204.1104. Invoices shall be submitted via Wide Area Work Flow Instructions listed in this solicitation. 12.
