- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number AG-04T1-S-14-0006 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2005-74. This is a small business set-aside with an associated NAICS code of 334210 and small business size standard of 1000 employees. This solicitation is for a supply of VOIP phones for the Umpqua National Forest Supervisor's Office located at 2900 NW Stewart Parkway, Roseburg, OR 97471. Please see schedule of items in Section B for complete list. This Supply of VOIP phones for the Umpqua National Forest is subject to Section 508 of the Rehabilitation Act of 1973 (29 U.S.C. 794d) as amended by the workforce Investment Act of 1998 (P.L. 105-220). See Section 508 - Accessibility of Electronic and Information Technology included with the schedule of items. The anticipated contract period is estimated to be September 1, 2014. PROVISIONS AND CLAUSES: See attached solicitation. QUOTE DUE DATE: Quotes shall be received by, August 15, 2014 at 1600 (4:00 pm PST). Quotes may be mailed to Umpqua National Forest, 2900 NW Stewart Parkway, Roseburg, OR 97471 or may be faxed to 541-957-3223. Note: prospective awardees MUST be registered in the System for Award Management (SAM) to be awarded a Federal Government Contract.
