- Synopsis/Solicitation Cover Letter Combined Synopsis/Solicitation Environmental Management System Flyer Past Performance Questionnaire Applicable Clauses and Provisions Performance Work Statement Pricing Schedule This contract is a personal services contract (PSC) and is intended to create an employer-employee relationship between the Government and the individual health care providers (HCPs) only to the extent necessary for providing the healthcare services required under this contract. The performance of healthcare services by the individual HCPs under a personal services contract is subject to day-to-day supervision and control comparable to that exercised over military and civil service HCPs engaged in comparable healthcare services. This requirement is in support of the 86th Medical Group's (MDG) Behavioral Health Optimization Program (BHOP). The contracted services shall include health consultations and health care facilitation in a specialty mental health clinic or in a patient centered medical home (PCMH) location, consultation services to the primary care managers (PCM), and delivery of behavioral health interventions in the primary care clinic that support PCM and patient treatment goals.
