- Attachment 7-Government Furnished Property Attachment 6-Confidentiality Statement Attachment 5-Wage Determination Attachment 4-Build Sheet Attachment 3-MOU Template Attachment 2-ISA Template Attachment 1-SOW RFP The Department of Health and Human Services (DHHS), the National Institutes of Health (NIH), the Clinical Center (CC), the Office of Purchasing and Contracts (OPC) has a need to procure dictation and transcription services. 100% Small Business Set-aside Competitive The NAICS code is: 561110. Potential offerors are responsible for monitoring Fed Biz Opps for the release of any subsequent amendments. Proposals shall include pricing information for a one (1) year base period and four (4) one year option periods. The Request for Proposal (RFP) and all attachments to the RFP can be found attached to this solicitation. QUESTIONS DEADLINE: Interested Offerors shall submit questions electronically to Colemanmn@cc.nih.gov and spolk@cc.nih.gov no later than August 5, 2014, 10:00 a.m. Eastern Standard Time. Companies that previously submitted proposal may do so again. Collect calls will not be accepted. No Phone Calls Please. PROPOSAL DUE: All proposals are due, electronically to spolk@cc.nih.gov no later than August 21, 2014, 10:00 a.m. Eastern Standard Time.
