- This RFQ is seeking quotes for nine laptop based Spirometers with the enclosed salient characteristics. Delivery Address: Robley Rex VAMC 800 Zorn Ave Louisville, KY 40206 Spirometer will perform all essential tests; FVC with Flow Volume Loop, Slow Vital Capacity (SVC), Maximum Voluntary Ventilation (MVV). In addition, Pre and Post Bronchodilator testing levels, and Bronchial Challenge Testing are also required. Software will provide user guidance and feedback regarding test quality according to ATS/ERS or other author's standards. Other software features necessary include report designer, 10 parameter-linked animated incentives, interactive guided calibration, customizable reference modules, and offline data entry. Include pricing for licensing subscription if applicable - Please provide specifications sheets or brochures (or links to the information). - Please specify if pricing is under contract (GSA) or Open Market - Indicate which item(s) by line number is open market, otherwise order will be processed as all items are under contract. - If item is under contract: Must provide Contract Number and Expiration date. - If shipping ORIGIN: Must indicate on Quote otherwise Purchase order will be processed as Destination - Preference: will be IAW FAR Part 13.003 (b)(1). - Requesting Service will review submitted bids as Technical experts to determine if technical solution is acceptable. - Bids will not be accepted after deadline. - All Questions must be submitted in writing not later than 7/23/2014, 12pm - All or none - At this time frame, no extension on closing date NOTE: All Offers and responses must be submitted by email to requester (see below).
