- Evertz Control Panels Request for Quote OC-393-2014-0107 This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a fixed priced purchase for (4) Evertz 2024E control panels, (3) Evertz 1000E control panels and (1) Evertz 2032E control panel. These panels will be located in the control room, audio booth, viewing station, and machine room. A purchase order award will be made within (5-10) business days after the response due of this notice. The North American Industry Classification Code (NAICS) is 541519. The Request for Quote (RFQ) is being set aside for Service-Disabled Veteran-Owned Small Business. Responses must be sent electronically in MS Word format to Lucille.Lee@cms.hhs.gov no later than 11:00am on August 6, 2014. Award will be made to the offeror with the lowest price technically acceptable. ALL QUESTIONS SHOULD BE SUBMITTED ELECTRONICALLY BY JULY 29, 2014. QUESTIONS WILL NOT BE ANSWERED BY TELEPHONE. PLEASE SUBMIT YOUR QUOTES IN WHOLE DOLLAR AMOUNTS ONLY.
