Description
VENDOR PACKET PEST CONTROL RFQ INTRODUCTION Homestead Job Corps Center is seeking quotes for Pest Control Services. THIS IS A SUBCONTRACTING OPPORTUNITY SCOPE OF WORK The contractor shall provide detailed, site-specific recommendations for any needed pest prevention. Contractor will be responsible for pest control inside and outside of 13 buildings as well as surrounding areas. Contractor will evaluate the pest management needs of the property and discuss with the Facilities Maintenance Supervisor. Contractor will also spray for mosquito control as needed. Contractor will be available for on call services such as, rodent control, bed bugs, beehives, crickets etc. At each visit, contractor will maintain a log of all services performed in all areas and is responsible for staff signatures in each area confirming service. This log must be submitted along with invoice. The contractor shall furnish all labor, tools, specialized equipment, material, supplies and transportation to perform services. PERIOD OF PERFORMANCE The period of performance for this project will be August 1, 2014- July 31, 2015. LOCATION The selected vendor for this project will perform the work at Homestead Job Corps Center, 12350 SW 285 Street, Homestead, FL 33033. REQUIREMENTS  Contractors must complete a Vendor Packet (provided by the Purchasing Agent). Interested parties must contact the Purchasing Agent to request Vendor Packet.  Contractors must be registered on https://www.sam.gov and be active without debarment from doing business with the government.  Contractors must provide proof of liability insurance with quote.  Homestead Job Corps Center is a federally funded facility and entitled to GSA pricing.  All fees and surcharges, if any, should be noted on quote or charges will not be paid if invoiced.  A site-visit is required in order to provide quote. Contact the Purchasing Agent to make arrangements.  The contractor and its employees must abide by all of the Center's rules and regulations. There shall be no fraternization between contractor and Homestead Job Corps Center students.  Payment terms are Net 30 days from date of invoice.  We reserve the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly.  This contract is subject to the provisions of the Service Contract Act (SCA) and all contractors must comply accordingly.  A single-source Blanket Purchase Agreement (BPA) will be awarded from this RFQ. A BPA number will be assigned and must be referenced on the invoice.  All prices are fixed for the duration of the contract and are not subject to escalation for any cause. Payment of the total fixed quote price shall constitute full payment for performance of the work and covers all costs of whatever nature incurred by the Contractor in accomplishing the work in accordance with the provisions of the contract.  This agreement is not intended by the parties to constitute or create a joint venture, partnership, formal business organization of any kind, or employer/employee relationship between the parties, and the rights and obligations of all parties shall be only those expressly set forth herein. Neither party shall have authority to bind the other except to the extent authorized herein. The parties to this agreement shall remain as independent contractors at all times, and neither party shall act as the agent for the other.  Subcontractor shall secure and keep current, at its own expense, all licenses and other certifications required by law or otherwise necessary to fulfill the statement of work. Subcontractor shall be solely and exclusively liable to third parties for all costs incurred by the subcontractor and for all claims of damage against the subcontractor arising out of or based on subcontractor's performance of this agreement, and is responsible for maintaining proper insurance, at the subcontractor's sole expense, to cover any and all such contingencies. Subcontractor shall also assume full responsibility for payment of any and all federal, state, and local taxes or contributions imposed or required under unemployment insurance, social security, and income tax laws, with respect to subcontractor's performance under this agreement.
