Description
The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), James J. Peters VA Medical Center, Bronx New York, Network Acquisitions and Logistics program is currently seeking firms, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hubzone, 8(A) OR Small Business with the following: Work Hours: Monday - Friday, 9:00 am 5:00 pm, excluding Federal Holidays. Location: VAMC Northport 79 Middleville Rd Northport, New York 11768 Office of Telehealth Services, bldg. 65, room 218 Equipment: Home Telehealth Devices: "Health Buddy (Bosch) "ViteNet (Vitel Care) "Viterion (Bosch) "Electronic House Call (Authentidate) "Commander Flex (Cardiocom) Task: The contractor shall: 1.Orders for monitors will be processed by their Customer Service Department. 2.Within 24 hours of receiving an order, a specially trained technician will notify the Veteran of the planned installation of the monitor, verify that the Veteran has received the monitor, and schedule an appointment. If needed, the Technician will pick up the monitor at the Northport VAMC and deliver to the Veteran's home at the time of the appointment. 3.The technician will provide specific instructions and educate to the veteran and/or caregiver on the operation of the equipment. A repeat demonstration should be performed and documentation of this will be completed and faxed to VA Telehealth. 4.The technician will install and test equipment as per manufactures procedures. 5.Product troubleshooting will be provided at the home as requested. Within 24 hours of notification. General Requirements: The contractor shall: a.Review/complete the on-line VAMC Contract Worker Orientation Handbook/Post-test and abide by all facility rules/regulations. b.Comply with all OSHA requirements as related to Performance of Work. c.Furnish and wear the contractor's own Photo Identification Badges. d.Complete Telehealth Device training (Health Buddy, ViteNet, Viterion,Electric House Call, Commander Flex and submit documentation of training by each vendor. e.The technician will dress in casual professional attire. Equipment Quality Assurance: The Denver Acquisitions and Logistics Center (DALC) will maintain a Quality Assurance Program for the Home Telehealth Equipment. Timeframe: All work performed outside of normal business hours shall be coordinated/approved in advance with the Lead Care Coordinator. Experience: The contractor shall be a Home Medical Equipment provider. The contractor shall have an insured, active, legitimate business in Home Medical Equipment and shall demonstrate practical experience working with this equipment. Documentation: The contractor shall provide the Office of Telehealth Services with documentation of all tasks. Justification: To use health information and telecommunication technologies to sensitively promote therapeutic relations. To enable shared decision-making between patient and practitioner across the continuum of care. To support patient self-management and an individual's responsibilities for their health. To provide an evidence-based platform to promote the health of the Veteran patient and Community. Firms desiring consideration shall email directly to the Contracting Officer referencing the following in the subject field, "Telehealth Equipment Support". Email should contain the following information: Name of firm, Socio-Economic category of the firm (SDVOSB, VOSB, 8A, HUBZONE or SMALL BUSINESS), complete address, phone & fax number of contact.
