- REQUEST FOR QUOTE Wage Determination 2005-2063_Attachment 2 STATEMENT OF WORK_Attachment 1 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This combined synopsis/solicitation is issued as a Request for Quote (RFQ) for F4D4A14142A002. Submit written quotes in accordance with (IAW) CLIN structure in announcement - Oral quotes will not be accepted. All firms or individuals responding must be registered in System for Award Management (SAM) (https://www.sam.gov). This is a 100% small business set aside. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-74 and DFARS DPN 20140528. North American Industrial Classification Standard (NAICS) 62111 and Size Standard of $10M apply to this procurement. Project Number: F4D4A14142A002 Project Title: RADIOLOGY TELE-MAMMOGRAPHY INTERPRETATION SERVICE 1. Please see attached Statement of Work (SOW) dated 17 July 2014 for detailed description of required duties. 2. Quotes shall be valid until 30 September 2014. 3. Period of Performance will be from 24 September 2014 through 23 September 2015, Base Year, with four (4) one-year options from 24 September 2015 - 23 September 2019 plus one six-month extension through 23 March 2020. 4. All questions or comments relating to this solicitation must be provided to the Contract Specialist (CS) and Contracting Officer (CO) in writing via email NLT 12:00 noon PST on 31 July 2014. 5. Quotes must be received no later than 12:00 pm PST on 15 August 2014 via attached Request for Quote (RFQ). 6. Quotes shall conform to the CLIN structure as established in this synopsis/solicitation.
