- These are the clauses the Government anticipates incorporating into the contract. This is the Statement of Work (SOW) document, which will be incorporated into the contract. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2014-69987 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: ITEM SUPPLIES / SERVICES QTY / UNIT UNIT PRICE EXTENDED PRICE 0001 Mathematical Modeler 1040 hours Base Year Period of Performance: 6/30/2014-5/31/2015 0002 Principal Investigator 208 hours Base Year Period of Performance: 6/30/2014-5/31/2015 0003 Other Direct Costs Dollars Base Year Period of Performance: 6/30/2014-5/31/2015 1001 Mathematical Modeler 1040 hours Option Period 1 Period of Performance: 6/30/2015-5/31/2016 1002 Principal Investigator 208 hours Option Period 1 Period of Performance: 6/30/2015-5/31/2016 1003 Other Direct Costs Dollars Option Period 1 Period of Performance: 6/30/2015-5/31/2016   All work shall be performed in accordance with the attached Statement of Work (SOW). Your quote shall include the following:  Total Price for CLINs 0001-0003 and 1001-1003  Capabilities in accordance with the Statement of Work and Evaluation Factors Period of Performance: 6/30/2014-5/31/2016 (including all option periods) Place of Performance: Centers for Disease Control & Prevention (CDC) 1600 Clifton Road Atlanta, Georgia 30329 (Specific instructions will be provided to Contractor at the time of award.)
