- Instructions to Offerors Provisions & Clauses SOW & Attachments SF 1449 NAICS: 238390 SIZE STANDARD: $14 Million IAW 52.219-6, Notice of Total Small Business Set-Aside (Nov 2011), this procurement is 100% set-aside for small businesses. Offers received from firms other than small businesses will not be considered. Please note, there is a mandatory pre-bid site visit. Refer to "Instructions to Offerors" for date/time/location. The Department of Health and Human Service, The Phoenix Area Indian Health Service (IHS) is soliciting quotes to reconfigure the Office of Health Programs (OHP) receptionist area and private offices located in the Indian Health Service's (IHS) Phoenix Area Office located at Two Renaissance Square, 40 N Central Ave, Suite 606, Phoenix, AZ, 85004. The work includes, but is not limited to, reconfiguration of cubicle and office furniture, workstations, cabinet installation, delivery, tear down, and removal. Additionally, the work is inclusive of all required electrical work. The OHP configured all office and open office cubicles with Herman Miller Action Office system furniture in calendar year 2010. Due to personnel changes and additional staff, the current configuration is not ideal. The OHP is seeking new system furniture and reconfiguration of existing furniture for the following areas: 1. Receptionist Area - System furniture and reconfigure receptionist area to accommodate two (2) administrative staffing areas. 2. Private Offices - New system furniture for four (4) offices, accommodating 2 staff work areas in each office.
