Combined Synopsis/Solicitation: CERTUS 140 2.45Hz Ablation System 1. GENERAL This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is VA261-14-Q-0715 and is issued as a Request for Quote (RFQ). No telephone requests for information will be accepted. This procurement is set-aside 100% Small Businesses and only qualified Offerors may submit bids. The applicable North American Industry Classification System (NAICS) Code is 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing and the Small Business Size Standard is 500 employees. OFFERORS MUST STATE THEIR BUSINESS SIZE when submitting their offer AND if the item is on a FSS/GSA schedule awarded to the Offeror, include the schedule information on your offer. A single award will be made as a result of evaluation of offers. New Equipment ONLY; NO remanufactured or "GRAY MARKET" items. All items must be covererd by the manufacturer's warranty. Offer must be good for 30 calendar days after the close of this Combined Synopsis/Solicitation. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. LINE ITEMS Manufacturer Part Number referenced: NeuWave Medical Item NumberDescription of Supplies/ServicesQtyUnitUnit PriceAmount 1Certus 140 2.45Hz Ablation System1EA Grand Total Product Specifications/Salient Characteristics: -Must have three (3) independent 2.45 GHz microwave generators, each capable of producing up to 140W each. -Must have one, easy to use, touch-screen user interface to control the system. -Must have up to 3 microwave ablation probes that can be connected to and be powered by the system at one time. -The maximum selectable power must be 140W when one probe is connected, 95Wper probe when 2 probes are connected and 65W per probe when 3 probes are connected. -Must have intermediate junction box or Power Distribution Module (PDMV) to reduce system set up complexity. -Probes must be provided sterile and intended for single patient use only. -Probes must be available for both percutaneous and open surgical applications. -Percutaneous probes must be comprised of 17 gauge needles, a probe handle, a 1.4M cable and a connector assembly. -Certus must be designed to perform optimally, in terms of efficiently transferring energy in tissue, in liver and kidney tissue. -Certus probes must be designed to perform optimally, in terms of efficiently transferring energy into tissue and in lung tissue. -All percutaneous probes must be available in 15cm and 20 cm lengths. -The open surgical probe, Certus must be 13 gauge and available in a 25cm length. -All probes must have temperature sensing thermocouples at various places along the shaft to monitor the probe temperature. -Must have a CO, based cooling system to ensure the non-active portion of the probe does not exceed temperature requirements. -The CO2 must enable the Tissue-Loc function, which can be used to adhere or stick the probe in place prior to starting ablation therapy. -The system must use two (2) F-sized CO, cylinders. When a tank in use empties, the system will automatically switch to using the other tank and notify the user to replace the empty tank. 3. DATE AND PLACE OF DELIVERY AND ACCEPTANCE AND FOB POINT Delivery must be within 30 days ARO FOB: Destination Deliver to: San Francisco Veterans Affairs Medical Center Warehouse/Bldg 203 4150 Clement St. San Francisco, CA 94121 4. APPLICABLE PROVISIONS AND CLAUSES The following FAR and VAAR provisions and clauses are incorporated into this solicitation document by reference: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (of which 52.222-3, 52.22219, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-1, 52.225-13 and 52.232-34 are applicable); and VAAR 852.246-70, Guarantee. Additionally, please include a completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. 52.212-2, Evaluation, Commercial Items and VAAR 852.211-73, Brand Name or Equal are also incorporated into this solicitation. At a minimum, the offered item must meet all requirements set forth in the product specifications/salient characteristics section to be considered a responsive offer. The Offeror that meets all the requirements in the solicitation, is responsive and responsible, and has the lowest cumulative price for all items (inclusive of delivery) will be selected. 5. OFFEROR ATTACHMENTS IF THE OFFEROR IS NOT THE MANUFACTURER OF THE QUOTED ITEMS, THE OFFEROR SHALL SUBMIT WRITTEN EVIDENCE AT THE TIME OF QUOTE SUBMISSION THAT CLEARLY VERIFIES THEY ARE AN AUTHORIZED DISTRIBUTOR AND/OR RESELLER OF THE QUOTED PRODUCTS and will provide the items on or before the specified delivery date. 6. OFFEROR INSTRUCTIONS The Government intends to issue a firm fixed price purchase order. Offerors must submit quotes electronically via e-mail. RESPONSES ARE DUE Tuesday, June 30, 2014 at 4PM EST.
