Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ-VA-251-14-Q-0373 is issued as a request for quote (RFQ) as a set aside for small business under NAICS code 811213. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. No telephone requests will be accepted. Only written or email requests received directly from the requester are acceptable. Line Item 1 - Contractor shall provide the following: -remove all existing televisions and mounts, -provide television splitters where necessary, -provide and install 22" flat panel televisions, -program all Rauland equipment for interface, -interface with pillow speakers for audio at bedside, EQUIPMENT Qty6 TV 6 ARTICULATING ARM MOUNT 2 TV SPLITTERS 6 REMOVE OLD TVS. 36 WIREMOLD 1 PROGRAM RAULAND 6 INTERFACE WITH RAULAND NURSE CALL 1 TEST 1 CLEAN UP 1 PROGRAMMING Line Item 2 - Contractor shall provide the following: -remove all existing televisions and mounts, -provide electrical service in 8 areas where electrical service is not near the new television location, -provide television splitters where necessary, -provide and install 22" flat panel televisions, -program all Rauland equipment for interface, -interface with pillow speakers for audio at bedside, -provide 42" television with input plat in lounge, -Three (3) additional Televisions for stock. Line Items 1 & 2 shall be provided to NIHCS, 2121 Lake Avenue, Fort Wayne, IN 46805. FOB Destination. Offorers must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses apply: 52.212-2, Evaluation-Commercial Item, the Government will award a purchase order resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. Price will be evaluated to determine its fairness, completeness, and reasonableness as it relates to the offer. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 4,7, 25, 27, 36, 37, 46, 48. Statement of Work for 4 East TV Replacements Northern Indiana Health Care NIHCS 2121Lake Avenue Fort Wayne, IN 46805 Scope of Work: Contractor shall provide and install the Televisions and associated TV mounts, articulating arm mounts, TV Splitters, electrical outlets, wiremolds, DVD player, and input panel listed in the table below to replace the existing 23 TVs and their TV mounts on the East Wing of the 4th Floor. Contractor shall also program the Rauland system so that all the TVs that will be installed in the patient rooms interface with the existing nursecall system at the VA NIHCS so that the patient can use the existing nurse call remote controller to control volume, mute sound, power TV on and off, change channels by using the channel up, channel down and number pad.
