Description
The purpose of this amendment is to extend the due date to June 17, 2014 @ 8:00 CST and provide answers to the following questions: 1.Please provide the square footage for all carpeted areas, tiled areas, canteen, retail and kitchen areas Answer: Carpet- None Tiled - All flooring is VCT, Ceramic, Quarry, etc. Canteen - 5719 Square Feet Retail Store - 2062 Square Feet Kitchen - 1177 Square Feet 2.If there is no pricing, and No Incumbent, how is the facility being maintained? Answer: Currently In-house staff. 3.Are the cleaners under the VA? Answer: Yes 4.If so would it be possible to interview them for employment at given time? Answer: No 5.In the Statement of Work (SOW) the language identifies Jefferson Ave but there does not seem to be any drawings associated with Jefferson Ave. Is the entrance located on Jefferson Ave? Is there an atrium? How tall is the glass? How far up is the cleaning required? Answer: The main entrance is located on Jefferson Ave. Yes, but the atrium is not associated with this solicitation. 6.Are any of the areas to be cleaned patient exam rooms? Answer: Yes 7.Is aseptic cleaning a requirement for this solicitation? Answer: Clinical and medical aseptic cleaning only. 8.Are there more floors other than two? The language suggests cleaning all windowsills and ledges daily. Is that correct? Answer: No 9.Will the government provide all equipment, supplies such as dust mops or disposable dust mops, vacuums, cleaning carts and floor strippers or buffers? Answer: No 10.Will the government hold the contractor harmless for marks or stains that are embedded in the carpet which cannot be removed without damage to the fibers? Answer: No, unless contractor is the person responsible for creating the stain. 11.Does the government want spot cleaning on only approved cleaning hours of 6am - 4:30pm only or will the government compensate contractors for after hour call backs. Answer: During approved cleaning hours. 12.Does one missed section negate the whole area or floor? Answer: No, but repeated occurrences would be considered. 13.Under the heading WEEKLY, how many wastebaskets are in Ambulatory Care and what size are they? Answer: 42 14.There seems to be an issue with walls being cleaned at this facility. If the walls are being cleaned on a weekly basis is there another form of cleaning that is required on a quarterly basis? Answer: Spot cleaning, removing stains, feces, urine, etc. daily. Complete scrub down and wash, top to bottom, weekly. 15. What is the square footage of the canteen, retail store and office space that needs cleaning six days a week? Answer: 6491 Square Feet 16.How many waste baskets are in the canteen that is required to be washed weekly? Answer: 15 17.Is an auto-scrubber sufficient for tile and grout cleaning? Answer: Yes 18.What is the square footage of the dining area? Answer: 5206 Square Feet 19.Are the hoods identified in the kitchen attached above stoves? How many hoods are there in the main kitchen? Answer: Yes, all 4 Hoods are attached. 20.Is the main kitchen separate from the canteen? If so what is the square footage of the main kitchen? Answer: Yes, 1177 Square Feet 21.How many windows are in the main kitchen? Answer: None 22.What type of flooring is there in the kitchen? Answer: Quarry Tile 23.Under the heading Building 1-2 Floor Hallway it appears that constant cleaning of these areas is required. Is that correct? Answer: Yes, extremely high traffic area. 24. Under heading Building 1- Restrooms page 4 are any of the public restrooms attached to patient rooms? Answer: No 25.On page 5 of the SOW. Is all trash considered trash from wastebasket only? Answer: No, all trash containers. 26. Will the government provide mops and rags to clean surfaces? Answer: No 27. Will the government provide housekeeping carts and all cleaning equipment (i.e. stripper, carpet cleaner, vacuums, buffers, etc.? Answer: No 28.Will the government provide posting area for inspection forms or can the contractor utilize binders? Answer: Permitted to utilize binders. 29.Does the rate of $11.16 include the health and welfare rate? Answer: No 30.Is it acceptable to give monthly cleaning price for areas that can be grouped or can the government simplify the daily, monthly and quarterly pricing? Answer: Yes 31.On the CLN 003 the government outlines mats but the scope of work does not address the mats. Does the government intend for the contractor to provide mat service or simply sweep / rinse off mats the government has at the site? Answer: Provide mat service. 32. Who is responsible for removing boxes? Are boxes included in the trash and will the contractor be responsible for recycling. Answer: Contractor will be responsible for box removal, and boxes are recycled in Our established single stream program. 33.Does the government bale boxes? Answer: No, they recycled in our single steam recycling program. 34.Are all VA departments (office areas) within the buildings required to remove their boxes to the recycling area? Answer: VA departments are only responsible for breaking down boxes. Boxes are then to be removed by the contractor to the recycling area. 35.Will the government provide housekeeping carts and all cleaning equipment (i.e. stripper, carpet cleaner, vacuums, buffers, etc.? Answer: No. Contractor is to provide floor care equipment (stripper, carpet cleaner, vacuums, buffers, polishers, and scrubbers, etc.).
