Description

SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to provide Emergency Housing services for homeless Veterans. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources to provide Emergency Housing services within 75 miles of the Lebanon VAMC location for the medical center's homeless Veterans population. Emergency Housing (10 to 25 beds) will need to be available on a 24/7 basis for requirement performance, and provide the following necessary services: case management, lodging and security, dining, laundry, and transportation. The source facility must be wheelchair-accessible and compliant with the American Disabilities Act (ADA) for handicap accessibility. The North American Industry Classification System Code (NAICS) is 624221 and the small business size standard is $10.0 million. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of Emergency Housing services for homeless Veterans. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 624221 and the small business size standard is $10.0 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.Provide a summary of experience and capability related to emergency housing services for homeless Veterans. This summary should include narrative which addresses the services of case management, lodging and security, dining, laundry, and transportation. 4.Provide a description of the credentials your facility staff possess to support homeless Veterans. 5.Provide a statement of the ability to provide emergency housing and support for homeless Veterans within 75 miles of the Lebanon VAMC location for the all underlined services described in this announcement. 6.If company holds a Federal Supply Schedule (FSS) Contract, please list the GSA Contract Number and relevant SINS. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
