Attachment 6 - US Department of Labor Wage Determination & Collective Bargaining Agreement Attachment 5- Relevant Past Performance Questionnaire Attachment 4- Contractor Staffing Change Report Attachment 3- Minimum Cleaning Frequency Requirements Attachment 2 - Building Information Attachment 1 - Federal Specifications Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Solicitation number is: OPM1914R0012 and is issued as a Request for Proposal (RFP). This solicitation document incorporates all mandatory commercial item provisions and clauses that are in effect through Federal Acquisition Circular (FAC) 2005-72 dated January 30, 2014. The North American Industrial Classification System (NAICS) code is 561720, and the small business size standard is $16.5 million. This procurement is being reserved exclusively for qualified 8(a) Small Businesses in accordance with FAR Part 19.8. The contractor shall provide all labor, incidental materials, and other services necessary to render services under the resulting contract, see Statement of Objectives detailed below. The period of performance is for five (5) years (base year and four (4) one-year periods). OPM intends to award one Firm Fixed Price Contract to the offeror that represents the best value to the government. The Period of Performance (POP) shall be five years (base year and four (4) one-year option periods). The base period will begin on or near the date of award. The anticipated award date is on or about June 15, 2014. Vendors not registered in the System for Award Management (SAM) database at the time the requirement is to be awarded will not be considered. Vendors may register with SAM by calling 1-866-606-8220, or by accessing www.sam.gov.
