Solicitation Number: N0016814RCHWA10 Notice Type: Sources Sought Synopsis: The Healthcare Acquisition Activity, Bethesda, MD, intends to issue a firm-fixed-price contract on a sole-source basis to CVTSA for the period of 1 January - 31 March 2014 for a group of Cardiothoracic Surgeons to provide the non-personal services of 24/7 on call effort and.25 FTE Cardiothoracic surgeon at Walter Reed National Military Medical Center (WRNMMC), Bethesda, MD. The period of performance from 1 Jan - 31 March 2014 The North American Industry Classification System (NAICS) for this requirement is 621111 and the size standard is $10M. All responsible sources able to provide the services required for the follow-on requirement, which includes your capabilities to develop and maintain a collaborative training relationship focused on Graduate Medical Education and development and maintenance of relationships with joint research activities {see sub-paragraph n) and o) of Section III. Specific Duties}, may submit a response to the Government point of contact via email. The follow-on contract is expected to be solicited during the 2nd QTR of FY 14 and awarded during the 3rd QTR of FY14. Sources are asked to indicate if they are a small business. A determination made by the Government to not compete this requirement may be based on responses to this notice and is solely within the discretion of the Government. Information received may be considered solely for the purpose of determining whether to conduct a competitive procurement. A solicitation may not be issued. Contracting Office Address: HCAA, NRCO, WRNMMC, 8901 Rockville Pike, Bethesda, MD 200889-5000 Description of Service: On-site cardiothoracic surgical assistance with cardiac cases of a highly advanced technical nature and support for 24/7on-call coverage with a 40 minute response time for emergency cases that may require mechanical support (ventricular assist device placement) as a life-saving measure. On-call HCP will provide on-site assistance in technically-advanced and also high-risk surgical cases, in addition to providing emergent support in cases of life threatening preoperative cardiac failure. The Contractor shall provide health care support for WRNMMC, Bethesda. The contract HCP will work in conjunction with other health care providers, professionals, contract and non-contract personnel. The quality of care provided by the Contractor HCP shall meet or exceed currently recognized national standards as established by the Joint Commission (JC). Contractor HCP shall provide highest regard to patient dignity and observe the precepts of the American Hospital Association's Bill of Rights for Patients. The Contractor HCP shall abide by the Medical Treatment Facility (MTF) rules, regulations and by laws, including Medical Staff By laws, as well as applicable Army regulations governing items like medical records. I. Qualifications And Experience Required: a)Graduation from a medical school approved by the Liaison Committee on Medical Education of the American Medical Association or the American Osteopathic Association or permanent certification by the Educational Commission for Foreign Medical Graduates (ECFMG). b)A current, valid unrestricted license to practice as a physician in anyone of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands and maintenance of same. The contractor shall comply with all applicable licensing regulations. All state medical licenses held by each Cardiothoracic Surgeon must be submitted as part of the credentials file. c)Certification in Thoracic Surgery as determined by the American Board of Thoracic Surgery, d)The group of surgeons will be required to have/be: 1.Performed at least 100 pump cases per year for a minimum of three years following completion of training. Results for each physician for the preceding 3 years shall meet or exceed current Society of Thoracic Surgeons predicted results for coronary artery bypass graft (CAB G), redo-CABG, valve replacement, and aortic root replacement. 2.At least one available surgeon shall have extensive experience in minimally invasive and robotic mitral valve surgery with at least 30 cases of this type per year for the previous 2 years. 3.Ongoing experience in ventricular assist device placement, with a minimum of 30-40 Ventricular Assist Devices (VAD) placed and managed by the group per year for the previous 2 years. 4.At least one available surgeon shall have ongoing experience in endovascular repair of the thoracic aorta, with at least 20 cases in the past year. 5.An active participant in an ongoing trial for trans-catheter aortic valve replacement. 6.At least one available surgeon shall have extensive experience in pediatric congenital heart surgery, with a minimum of 30 cases per year for the previous 2 years. 7.At least one available surgeon shall have extensive experience in minimally invasive aortic valve surgery, with at least 30 cases of this type per year for the previous 2 years. 8.At least one available surgeon shall have extensive experience in aortic valve-sparing root replacement surgery, with at least 10 cases of this type during the previous 5 years. II. General Duties: Contractor shall provide a full range of Cardiothoracic Surgeon Services. Scheduled Patient Treatment will be a function of the demand for the Cardiothoracic Surgeon Services as defined and described below. a)Provide Cardiothoracic Surgeon 24/7 on-call services b)Promote preventive and health maintenance care and positive health behaviors through education and counseling. Demonstrate awareness and sensitivity to patient/significant others' rights, as identified within the institution. c)Demonstrate awareness of legal issues in all aspects of patient care and unit function and strive to manage situations in a reduced risk manner. Participate in performance improvement activities. d)Demonstrate appropriate delegation of tasks and duties in the direction and coordination of healthcare team members, patient care, and clinic activities. e)Provide training as applicable to supporting Government employees (i.e., hospital corpsmen, students, etc.). f)Maintain an awareness of responsibility and accountability for own professional practice; participate in continuing education to meet own professional growth. g)Attend annual renewal of hospital provided annual training e.g. family advocacy, disaster training, infection control, sexual harassment, and blood-borne pathogens. h)Participate in monthly meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. Should a meeting occur outside of the healthcare worker's regular working hours, the healthcare worker shall be required to read and initial the minutes of the meeting. i)Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting. j)Perform limited administrative duties which include maintaining statistical records of the healthcare worker's workload, operate and manipulate automated systems such as CHCS or AHLTA, and participating in clinical staff quality assurance functions and Process Action Teams, as prescribed by the MTF Commander or his/her authorized representative. k)HIPAA Compliance. The contractor shall comply with the HIPAA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facility. Providers shall obtain/maintain a National Provider Identifier (NPI) in accordance with DOD and MTF policy instruction as applicable. III. Specific Duties: a)The contractor shall provide one cardiothoracic surgeon for primary on-call services 24 hours per day, 7 days per week. b)Provide pre-operative care, operative care, post-operative care and procedures. c)Serve as either the primary or assistant surgeon in cardiothoracic cases. d)Provide consultations and stand-by services for angioplasty procedures. e)Provide preoperative management of patients, to include clinical consultation, selection and timing of operative intervention, and the selection of operative procedures. f)Provide surgical care for patients to include open and closed heart operations and operations on the great vessels of the chest. g)Provide both inpatient and outpatient consultative opinions for members of the cardiothoracic surgery staff upon request during scheduled, in-house, on-site coverage. h)Direct and provide appropriate diagnostic and therapeutic measures for inpatients. i)Maintain a cohesive working interaction with the Cardiology Division and Pulmonary Disease Division of the WRNMMC. j)Assist the department in conducting their training programs in thoracic and cardiovascular surgery. k)Assist in the provision of post-operative management of cardiothoracic surgery patients. l)Provide clinical/technical advice and/or periodic training to members of the WRNMMC staff. Topics will include, but not be limited to cardiothoracic surgery procedures, developments, or related issues. This includes providing the actual training and completing the training evaluation. m)Provide collaborative services and case experience to the staff military surgeons, with regard to advanced surgical techniques, including minimally invasive and robotic cardiac surgical procedures, trans-catheter valve replacement, ventricular assist device placement, and hybrid cardiac procedures. These services may require staff military surgeons to travel to the contractor's hospital for observation and participation in these types of procedures, at a schedule to be determined by the two parties. n)Additionally, develop and maintain a collaborative training relationship focused on Graduate Medical Education (GME), with WR residents rotating at local hospitals centered on hospital service in surgery, anesthesia, and cardiology. This training requirement includes oversight and consultation with residents involved in cardiothoracic surgical procedures, perioperative patient care and management, with particular emphasis on heart failure interventions and care, along with participation in didactic conferences on a weekly basis. o)Joint research activities relationships will also be developed and maintained. This includes collaboration in both site-specific and multi-institutional studies involving data-gathering, results analysis, and review of scholarly writings. Assistance in grant-writing will also be provided, as needed. IV. COMPETENCIES 1 KNOWLEDGE COMPETENCIES. WR residents will demonstrate factual knowledge of: i. Performance criteria for competencies described above in Section I entitled quote mark Qualification and Experience Required quote mark and administrative and clinical standards. ii. Service practice standards and all policies and procedures related to training. (2) CLINICAL COMPETENCIES. WR residents will demonstrate skill in applying the competencies described above in Section III entitled quote mark Specific Duties quote mark. (3) TEACHING COMPETENCIES. (4) ADMINISTRATIVE COMPETENCIES. (5) FINAL Certification. WR Residents will designate individuals as certified providers.
