- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation is available on the AFSI website. Solicitation number W912PQ-14-T-0002 is being issued as a Request for Quote. This procurement is unrestricted. The cost magnitude is estimated between $750,000 and $2,000,000 per year. This is an annual contract with a Period of Performance of 1 Jan 2014 - 31 Dec 2014. Mobile Dental Services are to be provided in support of Individual Medical Readiness Including Exams, Treatment and DENCLASS Entry. Contractor shall provide management, labor, material, equipment, certifications and supplies necessary to provide all listed services utilizing a HIPPA Privacy Act, OSHA and ADA Compliant mobile health facility or portable equipment in accordance with the Statement of Work which is attached to and made part of this solicitation.
