Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-14-Q-0171is issued as a (Request for Quotation (RFQ). This requirement is Small Business. NAICS: The NAICS is 562998 and the small business size standard is $7 million. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. To be eligible to receive a government award, the offeror must be currently registered in System for Award Management or may register with SAM at http://www.sam.gov/ Period of Performance (POP): Base Year - December 15, 2013 to September 30, 2014 Option 1 - October 01, 2014 to September 30, 2015 Option 2 - October 01, 2015 to September 30, 2016 Option 3 - October 01, 2016 to September 30, 2017 Option 4 - October 01, 2017 to September 30, 2018 Description of Services: Contractor shall provide all labor, materials, tools and special equipment to pump and dispose of grease and residues from a 1,500 gallon grease trap located near the Loading Dock at the VA Medical Center, Memphis, TN. Cleaning of the grease trap shall occur on a monthly basis. Time and date of plumbing shall be scheduled in advance with the Plumbing shop supervisor. Pumping for this pit shall be scheduled between the hours of 6:00am and 7:00am, Monday through Friday only, excluding Federal government holidays. Contractor shall be responsible for leaving the area in clean condition. This service is required for the Memphis VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104. THE CONTRACT PERIOD IS FROM 12-15-13 TO 9-30-14. See Statement of Work (SOW) for complete details. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation. To receive consideration as SDVOSB or VOSB, the firm must be registered and verified in the VIP database at www.vip.vetbiz.gov. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable shipping charges, completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. Quotes must be received by December 13, 2013 by 9am CST. All quotes must be submitted by electronic submission in reference to VA249-14-Q-0171.
