- Combined Synopsis/Solicitation Exhibit N - Historical Data for FY 2012-2013 Wellness Reduction Schedule Exhibit M - Historical Data for FY 2012 Wellness Reduction Schedule Exhibit L - Map of Complex and Trash Receptacle Area This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 in conjunction with FAR Part 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 483-13MR-7381 and is being issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-71. This is a Small Service Disabled Veteran Owned business set-aside with a NAICS code of 713940, Fitness & Recreational Sports Center with a small business size standard of $7 million. The provision at 52.212-1, Instructions to Offerors-Commercial and Addendum to 52.212-1 applies to this acquisition. FAR 52.212-2 and Addendum to FAR 52.212-2 applies to this acquisition. See attached combined synopsis/solicitation for complete solicitation and evaluation criteria. Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. FAR Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. See attached combined synopsis/solicitation for complete clause and additional FAR clauses within 52.212-5 that are applicable to this acquisition. A site visit will be held for this requirement.
