- REVISED SCOPE OF WORK AMENDMENT 1 THIS AMENDMENT IS ISSUED TO REVISE THE SCOPE OF WORK AND THE CARPET TILE WEIGHT TO 32 OZ/SQUARE YARD AND ALSO SPECIFY THAT THE CARPET BE CLOSED LOOP. THE BID OPENING DATE WILL BE EXTENDED FROM AUGUST 27, 2013 TO SEPTEMBER 4, 2013. Provide construction services to replace existing carpet in the Whiteriver Hospital located at 200 West Hospital Drive, Whiteriver, Arizona 85941. Carpet replacement in the Hospital is estimated at approximately 5,000 square feet (556 square yards). Building and room locations and dimensions are indicated on the attached project drawings. The construction services shall include removal of existing carpet, surface preparation of floors and field installation of new carpet tile as selected by the facility manager. The contractor shall be responsibler for all equipment, parts, material, labor, travel, per diem, TERO fees, and other expenses required to complete the project as described in this scope of work.
