- SOW Request For Quote Wage Determination This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR part 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation, which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-69. This acquisition is set-aside for Small Businesses in accordance with FAR part 19.5. The associated NAICS code for this acquisition is 811490 with a size standard of $7 Million. The contractor shall provide all personnel, equipment, tools, supplies, materials, supervision and transportation necessary to provide an on call technician to operate, maintain, and repair the fitness equipment in accordance with the attached Statement of Work, dated 19 June 2013. The following is a description of the items as detailed in the attached solicitation. Base Year CLIN 0001 - (12 Months) Fitness Equipment Maintenance CLIN 0002 - (9,200 lots) Fitness Equipment Maintenance Parts Draw (Not to exceed $9,200.00) Period of Performance shall be 01 October 2013 - 30 September 2014 Option Year 1 CLIN 1001 - (12 Months) Fitness Equipment Maintenance CLIN 1002 - (9,200 lots) Fitness Equipment Maintenance Parts Draw (Not to exceed $9,200.00) Period of Performance shall be 01 October 2014 - 30 September 2015 Option Year 2 CLIN 2001 - (12 Months) Fitness Equipment Maintenance CLIN 2002 - (9,200 lots) Fitness Equipment Maintenance Parts Draw (Not to exceed $9,200.00) Period of Performance shall be 01 October 2015 - 30 September 2016 Option Year 3 CLIN 3001 - (12 Months) Fitness Equipment Maintenance CLIN 3002 - (9,200 lots) Fitness Equipment Maintenance Parts Draw (Not to exceed $9,200.00) Period of Performance shall be 01 October 2016 - 30 September 2017 Option Year 4 CLIN 4001 - (12 Months) Fitness Equipment Maintenance CLIN 4002 - (9,200 lots) Fitness Equipment Maintenance Parts Draw (Not to exceed $9,200.00) Period of Performance shall be 01 October 2017 - 30 September 2018 The following FAR Provisions/ Clauses are included in the attached solicitation: FAR 52.212-1, Instruction to Offerors. Far 52.212-2, Evaluation- Commercial Items FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. FAR 52.212-4, Contract Terms and Conditions-Commercial Items FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. FAR 52.232-18, Availability of Funds (Apr 1984), Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. Additional FAR Provisions and Clauses are included in the attached solicitation. Award will be made to the responsive offeror determined to be the most advantageous to the government in accordance with the criteria listed in the attached solicitation. Awardee will be required to to invoice through wide area work flow (WAWF). Award will only be made to contractors registered in System for Award Management(http://www.SAM.gov) Please fill out the attached Request For Quote and send with your offer.
