- Solicitation document The Centers for Disease Control and Prevention (CDC), National Institute for Occupational Safety and Health has a requirement for "A Healthcare Organization to Pilot Capture of Patient Industry and Occupation Data into an Electronic Health Record." This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. This solicitation is being issued as a Request for Quotation (RFQ) as number 2013-N-15738R using FAR Subpart 13 Simplified Acquisition Procedures. Any incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69, effective August 1, 2013. This RFQ is unrestricted. The North American Industrial Classification System Code is: 621498 with a small business size standard of: $9 million. Offerors may fill in the dollar amounts for each CLIN listed below or provide a separate document containing the same information: CLIN 0001: Total Price EHR & O Pilot Study (Base Period) (18 months) $________ CLIN 0002: Total Price Deliverables (Option 1) (12 months) $________ (All work shall be performed in accordance with the attached Statement of Work) Your quote shall include the following: • Separate Prices for CLIN 0001 and CLIN 0002 • Capabilities and experience in accordance with the Statement of Work and Evaluation Factors See attached document(s) for Statement of Work, Addendum, Clauses, detailed solicitation, Contractor Performance Report and other additional requirements/items. QUOTERS ARE ADVISED THAT AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the quoter's best terms from a technical and price standpoint. The technical proposals shall not exceed 10 pages. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. Quotes are due on August 28, 2013, by 2:00 PM EDST and should be sent to the attention of Cynthia Y. Mitchell via email at cmitchell@cdc.gov. Hard copies can be delivered via Fed Ex or UPS to Cynthia Y. Mitchell, Centers for Disease Control and Prevention, Procurement and Grants Office, 626 Cochrans Mill Road, Pittsburgh, PA 15236. The Offeror must cite RFQ Number 2013-15738RCM in the email subject line and on the shipping label if sending hard copies. Evaluation and award of the purchase will be performed in accordance with FAR 13.106-2 Evaluation of Quotations or Offers and FAR 13.106-3 Award and Documentation. The Offeror must have an active registration with the System for Account Management (SAM), which has replaced the Central Contractor Registration (CCR), prior to award of the purchase order in accordance with FAR Clause 52.204-7. The government intends to issue a firm, fixed price purchase order in accordance with FAR Part 13. After evaluation of the following technical factors, award will be made to the lowest priced technically acceptable offer. Each offer will be evaluated on a pass/fail basis, so offerors are cautioned that they should submit responses in sufficient detail to enable the government to evaluate their capabilities and experience. Electronic quotations will be accepted. All prospective offerors interested in submitting a quote must include company name, mailing address, and point of contact, phone number, e-mail address, business status, and information as required above.
