- QASP Performance Work Statement (PWS) Combined Synopsis/Solicitation GENERAL INFORMATION The 42d Contracting Squadron at Maxwell AFB, AL has a requirement to purchase services as described in this document. This acquisition will be procured using Simplified Acquisition Procedures prescribed under FAR 13.5 Test Program for Certain Commerical Items. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal (RFP) and the reference number is FA3300-13-R-0014 Special Needs Coordinator-Registered Nurse. This solicitation has incorporated provisions and clauses in effect through Federal Acquisition Circular 2005-69 ( Effective 1 Aug 2013), DPN 20130808 ( Effective 08 Aug 2013), and AFAC 2013-0327 (Effective 27 Mar 2013). A firm-fixed price contract for personal services will be made in accordance with the following: This acquisition is a 100% set-aside for small businesses. The NAICS Code is 621399 Office of All Other Miscllaneous Health Practitioners with a small business size standard of $7 million, the Federal Supply Code (FSC) is Q401. The Defense Priorities and Allocations System rating is S10. NATURE OF ACQUISITION Personal Services to provide one (1) Full Time Equivalent Registered Nurse(RN)-Special Needs Coordinator (SNC) to oversee and manage the Exceptional Family Member Program by establishing and maintaining procedures to identify sponsors whose family members have special medical and educational needs in accordance with (IAW) the Performance Work Statement (PWS). SCHEDULE OF SUPPLIES/SERVICES: Maxwell AFB has a requirement to purchase the below listed services. This procurement will be awarded under Simplified Acquisition Procedures. All inquiries/responses to this notice must be received by 16 September 2013, 11:00 hrs CST. CLIN 0001 : PERSONAL SERVICES: To provide one(1) full-time equivalent Special Needs Coordinator- Registered Nurse to oversee and manage the Exceptional Family Member Program IAW the Performance Work Statement (PWS).
