- VA247-13-I-1051 - SOURCES SOUGHT NOTICE THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs, Charlie Norwood Veterans Affairs Medical Center (CNVAMC), in Augusta, GA is conducting a MARKET SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to SIN Code: 874 507 - "Operations & Maintenance Logistics Management and Support Services" or NAICS562111 for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable SIN: 874 507 - "Operations & Maintenance Logistics Management and Support Services" or NAICS 562111 should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision and equipment necessary to provide services according to the Statement of Work. NAICS: 562111 - "Solid Waste Collection" OR GSA/FSS Description: - Logistics Worldwide / SIN: 874 507 - Operations & Maintenance Logistics Management and Support Services DESCRIPTION OF THE REQUIREMENT: 1)Scope or Mission: a.Provide service to pick-up, haul to landfill, empty, clean, sanitize, disinfect and replace Government-owned 42 cubic yard dumpster compaction container, Uptown Division, in accordance with specifications of A.6, Dumpster Compaction Containers Weekly pulls are required. b.Provide rental, collection, empty, haul to landfill, clean, sanitize and disinfect eleven (11) 8- cubic yards dumpsters, in accordance with A.7, Dumpster Containers and C.1, Frequency of Collection Schedule weekly pulls are required. c.Provide pick-up, haul to landfill, empty, clean, sanitize, disinfect and replace Government-owned 42 cu. yd. self-contained dumpster compaction unit which includes container, Downtown Division, in accordance with specifications - See A.6, Dumpster Compaction Containers service is required twice a week. d.Provide Rental, collection, empty, haul to landfill, clean, sanitize and disinfect one (1) 30 yard roll off dumpster for yard waste, Downtown Division, in accordance with specifications - see A.8, Roll-Off Dumpster. Pulls will be required no more than six times annually. Cost to include Landfill e.Provide Rental, collection, empty, haul to landfill, clean, sanitize, and disinfect six (6) 96 gallon containers, Uptown Division - Fisher House, twice a week. 2)Period and Place of Performance: Contract will be performed on the campus of the Charlie Norwood VAMC Uptown, Downtown, and the Fischer House. Requirement for performance will be from October 1, 2013- September 30, 2014. 3)Operating Constraints: Removal/ emptying containers must be scheduled Monday - Friday between the hours of 6:30am-2:30pm. STATEMENT OF WORK FOR SOLID WASTE COLLECTION/REMOVAL VAMC AUGUSTA, GA A.3SOLICITATION The solicitation adheres to the format defined in FAR 12.303 for the acquisition of commercial products. A.4TYPE OF CONTRACT The VA Medical Center will award a firm fixed price contract pursuant to this solicitation. A.5GENERAL REQUIREMENTS The contractor shall furnish all labor, transportation, materials, equipment and supervision required to provide refuse collection services for both divisions of the Department of Veterans Affairs Medical Center, Augusta, Georgia. A.6DUMPSTER COMPACTION CONTAINERS A.6.1 The contractor shall pick-up, haul to landfill, empty, clean and sanitize, disinfect and replace Government-owned dumpster compaction containers: a. Uptown Division, Bldg. 111 - 42 cubic yard container shall be emptied on Thursday between 8:00 AM and 9:00 AM. b. Downtown Division, Bldg. 801 - 42 cubic yard container shall be emptied on Tuesday and Friday between 7:00AM and 11:00 AM. A.6.2 An alternate container is located adjacent to the compaction units and shall be put in place of full unit immediately following pick-up. Upon return from the landfill, the freshly emptied and cleaned units shall be placed adjacent to the compaction unit for use after the next pick-up. A.6.3 The contractor shall be responsible for any damage to the compaction units, containers or VA property during the moving or hauling of containers to and from the landfill. The contractor shall report damages immediately to the VA Police and Security or the Environmental Division of Facility Management or Contracting Officer. A.6.4 The contractor shall provide Environmental Division with monthly certified landfill weights from all compactor loads. A.6.5 For all visits by Contractor: Contractor (or representative) shall sign in at Environmental Division Office, Bldg. 110, Room GD111, 706-733-0188 ext. 7559 for Uptown Division and Bldg. 801, Room 1B173, 706-733-0188 ext. 3319 for Downtown Division for items listed under contract. Contractor shall also sign out before leaving and present service ticket for all work performed. Failure to follow these procedures may result in delay of payment or non-payment. A.7 DUMPSTER CONTAINERS A.7.1 The contractor shall provide eleven (11) each eight (8) cubic yard dumpsters identified on "Frequency of Collection Schedule (See Section C, paragraph C.1). The contractor shall clean and maintain these boxes in accordance with conditions outlined. Dumpsters shall be emptied in accordance with "Frequency of Collection Schedule." A.7.2 All dumpsters, located at both Uptown and Downtown Divisions, shall be emptied by 7:30 AM. A.7.3 Refuse Compaction at bottom of dumpsters: The contractor shall remove, as needed, any compacted refuse on the inside of all dumpsters. Compacted material of no more than two (2) inches shall be allowed in boxes. A.8 ROLL-OFF DUMPSTER Contractor to provide one (1) each 30-yard roll off dumpster for yard waste. Dumpster to be located at designated location, Downtown Division. In addition, contractor will collect, empty, haul to landfill, clean, sanitize and disinfect unit. Approximately five (5) tons per haul/pick-up are anticipated. Landfill disposal charges are included in service. A.9 ADDITIONAL SPECIFICATIONS: A.9.1 Pick-up On Holidays: The contractor shall provide service on Federal holidays if the schedule pick-up falls on a holiday. Federal holidays include New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas, and any other day specifically declared by the President of the United States to be a national holiday. A.9.2 Saturday and Holiday Collection, Downtown Division: Veterans Affairs (VA) Police shall be contacted on Saturdays and holidays to open the gates to the courtyard where dumpsters are located. VA Police may be contacted by telephone, 706-733-0188, ext. 3211, or in person at the Outpatient Entrance to the Downtown Division, Room 1D202. A.9.3 Contractor shall pick up trash refuse which falls from the container while being loaded. Refuse not inside box shall not be the contractor's responsibility unless items are released from box as the result of contractor's moving/dumping action. Such items shall be picked up prior to contractor continuing performance. A.9.4 Odor in containers shall be controlled by the application of chemical brand "Suppress" or equivalent. The contractor shall provide Environmental Division (ED) with Material Safety Data Sheets (MSDS) prior to use of any chemical. If chemical is changed during the contract period, the contractor shall provide ED with MSDS at least ten (10) days prior to change. Odor control to be performed on all containers after each pick-up. A.9.5 Contractor shall provide additional pick-up of compaction containers and dumpsters, if needed, within twelve (12) hours of call when initiated by ED personnel, at established contract cost. A.10 SAFETY REQUIREMENTS In the performance of this contract, the contractor shall take such safety precautions as the contracting officer or his/her designee may determine to be reasonably necessary to protect the lives and health of occupants of the buildings. The contracting officer or his/her designee will notify the contractor on any non-compliance with the foregoing provisions and the action to be taken. The contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforementioned. If the contractor fails or refuses to comply promptly, the contracting officer may issue an order, stopping all or any part of the work and hold the contractor in default. A.11 SPECIAL SERVICES Payment for surcharges or special fees levied by the Richmond County Landfill Facility to the contractor for disposal of special/biomedical waste will be made only after receipt of documentation from the landfill facility to verify cost of the special service. The contractor shall immediately notify the contracting officer's designee when biomedical waste is found in the containers. (Note: Yard waste covered in Line Item #4). A.12 CONTRACT ADMINISTRATION DATA A.12.1 The contracting officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes which will affect price, quantity, or quality of performance of this contract. In the event the contractor effects any such change at the direction of any person other than the contracting officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. A.12.2 852.270-1 Representatives of Contracting Officers (APR 1984) The Contracting Officer reserves the right to designate representatives to act for him/her in furnishing technical guidance and advice or generally supervise the work to be performed under this contract. Such designation will be in writing and will define the scope and limitations of the designee's authority. A copy of the designation shall be furnished to the contractor. A.13 NOTICE OF INTENT This is to inform you that in accordance with the Metric Conversion Act of 1975 (Public Law 98-168), the Omnibus Trade and Competitiveness Act (Public Law 100-418), Executive Order 12770, and VA Directive 0010, our VA Medical Center has every intention to convert and implement procurement, grants, and other business related measurements to metrics by September 30, 1999. During the transition period, VA will use metric system measurements in all business related activities (unless metric usage is impractical or would have an adverse impact on the market share of U.S. firms). Business related activities include, but are not limited to, computer data files, medical, nutrition, travel, forms, weights, measures, temperatures and signage. QUALIFICATIONS OF CONTRACTORS: A.Contractors shall ONLY respond if they currently have the equipment and personnel that meets all of the specifications for trash removal. D.A statement indicating the type of business, i.e. other than small business or small business (by type), if services identified meet all specifications in the Statement of Work and provide current GSA Schedule (FSS Contract) and provide reference data if yes. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Potential sources shall provide (during demonstration if accepted) documentation of successful clinical outcomes by trial or case study; preferably VA based. If VA based information is provided, also provide VA reference contact information. 3) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of product/service. Please include the following: (a) staff expertise including their availability, industry experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and large project planning capability; and (e) examples of prior completed Government contracts and references (preferably VA), and other related information. 4) Submit Capability Statements by 2:00 pm Eastern Time Zone, Wednesday, July 31, 2013 to: Department of Veterans Affairs, VA Southeast Network - VISN 7 ATTN: Tanjia Harris, Augusta, GA 30901. Responses should be received no later than 2:00 PM EST. Submissions must be made via email to: Tanjia.Harris3@va.gov or uploaded to GSA. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER JULY 31, 2013. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION).
