- THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A SOURCES SOUGHT NOTICE for the identification of potential vendors who have the capacity to provide BRCA (Breast Cancer) Testing, Hereditary Cancer Panels. These services include genetic testing for breast oncology when needed to make diagnostic, management, and secondary prevention decisions. Requirements: "Have CAP/CLIA licensed/approved laboratory with verifiable/demonstrated experience/expertise in BRACAnalysis Testing. This requirement must be substantiated by past performance in the field of reference testing for these specialties. "Healthcare laboratory that is covered under the Health Insurance Portability and Accountability Act (HIPAA) enacted in 1996 and is subject to the regulations set forth by the law. "Requires all employees to undergo privacy awareness training and has implemented administrative and technical safeguards to protect patient privacy. "Perform BRACAnalysis ® Testing to assess a woman's risk of developing hereditary breast or ovarian cancer based on detection of mutations in the BRCA1 and BRCA2 genes. "Provide technical and professional services for BRACAnalysis Testing. "Provide a test request form with a statement of medical necessity. "Provide at no additional cost to the patient or physician: transport containers; telephone counseling/consultation with reference laboratory's pathologist when deemed appropriate. "Reports must include comments that reflect clinical application. Delivery: "Reports should be faxed to Bay Pines Pathology and Laboratory Medicine Office at 727-319-1075 and must be followed by a mailed report to Bay Pines VAHCS, 10,000 Bay Pines Boulevard, Building 100, Second Floor Lab, Room 2B-110, Bay Pines, Florida 33744. "Pathologist reviews all results and integrates information into diagnosis. "Reports must be provided within 14 business days of specimen receipt. "Bay Pines VA Pathology & Laboratory Medicine will be responsible providing payment within a reasonable amount of time. See attached Statement of Work following this announcement. The North American Industry Classification System code (NAICS) is 621511 and the size standard is $30.0 (in millions of dollars). All interested and capable respondents are requested to provide the following requested information. All responses to this notice shall include: company name, address, point of contact, phone number, DUNS number (www.dnb.com), confirmation they can provide and service, and to perform and report the required BRCA1 and BRCA2 testing. Telephone responses wi07/ll not be accepted. Reponses should be sent via e-mail to Janice.fornaro@va.gov not later than 5:00 PM EST, July 25, 2013. All contractors interested in doing business with the Government must be registered in the System for Award Management (SAM) database.
