- This is a SOUGHT NOTICE to determine the availability and capability of potential small businesses (e.g., 8(a), service-disabled veteran-owned small business, HUBZONE small business, small disadvantaged business, veteran-owned small business, and women-owned small business) in accordance with FAR 7.102(s)1. The potential small business will need to demonstrate the ability to (1) Implement the Blueprint through Developing State Community of Practice Collaboratives (2) Expand State Participation in the Association of State and Territorial Public Health Nutrition Directors (ASTPHND's) Maternal and Child Health Nutrition Council (MCH). BACKGROUND: The Department of Health and Human Services, Health Resources and Services Administration (HRSA) is the primary Federal agency tasked with improving access to health care services for underserved, uninsured, isolated and medically vulnerable. HRSA's mission is to improve health and achieve health equity through access to quality services, a skilled health workforce, and innovative programs. In alignment with HRSA's strategic plan goal to expand the use of more preventive health care to reduce serious illnesses, like obesity, diabetes, and heart disease, a significant concern from a public health perspective is that overweight children grow up to be overweight or obese adults and thus are at greater risk for some major health problems, such as heart disease and diabetes. Much work remains to meet the Healthy People 2010 nutrition and physical activity targets. DESCRIPTION OF WORK: The NAICS code is 541690. The contract will include the following requirements: 1. The contractor shall facilitate leadership workforce development in their membership through working with the six (6) Association of State and Territorial Public Health Nutrition Directors (ASTPHND) MCH Nutrition training programs to form a minimum of three (3) regional networks throughout the continental United States. The contractor shall submit plans for development of ASTPHND's role in each regional network and provide technical assistance to the States as needed. 2. The contractor shall provide technical assistance to states to implement the nutrition components of the Affordable Care Act. CONTRACTOR REQUIREMENTS: •The contractor shall provide technical assistance to existing state programs to implement the Blueprint through supporting State Community of Practice Collaboratives. •The contractor shall select a minimum of three (3) states in which to implement the Blueprint collaborative on an annual basis. The contractor shall choose additional states for the option years. •The contractor shall develop a draft plan to implement the Blueprint Community of Practice in each of the selected states. Each plan shall include technical assistance on meetings to be conducted by conference calls. •The Contractor shall develop a plan to expand the number of state participants in the ASTPHND's Maternal and Child Health Nutrition Council (MCH). The plan shall include goals, objectives, activities, timelines, and evaluation with a focus on the Affordable Care Act •The contractor shall convene quarterly conference calls for the ASTPHND's MCH Nutrition Council, focusing on Work Force Development and the Affordable Care Act. The contractor shall deliver: A) Selection of States to Implement Blueprint (MS Word -1 electronic copy) B) Draft State plans (MS Word -1 hard copy/1 electronic copy) C) ASTPHND's MCH Nutrition Council Plan (MS Word -1 electronic copy) D) Plan for technical assistance and development of the ASTPHND's MCH Council (MS Word-1 electronic copy) E) Electronic Progress Reports (MS Word -1 hard copy/1 electronic copy) F) Annual Copy of report ( MS Word -1 hard copy/1 electronic copy) Interested parties that have capabilities necessary to perform the stated requirements may submit capability statements to Biji John at Bjohn@hrsa.gov. CAPABILITY STATEMENTS MUST DEMONSTRATE THE MINIMUM REQUIREMENTS OUTLINE ABOVE. Please address each in the order listed above. TEAMING ARRANGEMENTS: All teaming arrangements shall include the above-cited information and certification's for each entity on the proposed team. Teaming arrangements are encouraged. Responses must be submitted no later than August 5, 2013, 10:00 PM EST. Capability statements will not be returned and will NOT be accepted after the due date. This is not an invitation for bid, request for proposal or other solicitation and in no way obligates HRSA to award a contract. The sole intent of this Sources Sought Notice is to obtain capabilities for set-aside and procurement planning purposes. Please provide the following Business Information: 1. DUNS Number 2.
