Description

THE PURPOSE OF THIS CHANGE IS TO INCREASE THE NUMBER OF ZOLL AEDS FROM 99 TO 102 AND REPLACE THE SCOPE OF WORK DATED 06/25/2013 WITH A VERSION DATED 07/09/2013. The Federal Aviation Administration (FAA), Mike Monroney Aeronautical Center (MMAC), Oklahoma City, OK, is issuing this Request for Quote (RFQ) for online access to the En-Pro and PlusTrac AED management system and provides uninterrupted access to the MMAC AED historical documents. The FAA will not pay for any information received or costs incurred in preparing the response to the request for proposal. Therefore any cost associated with the request for proposal submission is solely at the interested vendor's expense. The contractor shall provide online access to the En-Pro and PlusTrac AED management system for 99 ZOLL AED ™s; provide complete medical direction regarding the use of AEDs; and continually monitor overall compliance and AED readiness in accordance with the attached Scope of Work (SOW). AEDs are Automatic External Defibrillators. The contractor will provide services for MMAC to maintain a Public Access Defibrillator (PAD) program in accordance with the Guidelines for Public Access Defibrillator Programs in Federal Facilities, Office of Public Health and Science, Office of the Secretary, HHS and Office of Government wide policy, GSA. The competition will be based on lowest price proposal meeting the standards outlined in the SOW. This competition is open only for small businesses, or service-disabled veteran-owned small businesses or eligible socially and economically disadvantaged businesses that are certified by the SBA for participation in the SBA's 8(a) Program. Contractors must be registered in the System for Award Management (SAM) database (www.sam.gov) prior to submitting a response. Vendors participating in the SBA's 8(a) Program must provide a copy of their SBA 8(a) certification letter. The NAICS Code for this requirement is 423450, Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers, with a Small Business Size Standard of 100 employees. Attached are the Scope of Work, Terms and Conditions and FAA Business Declaration Form. Please respond by reviewing these items, and submitting a completed, signed quote with complete contact information and a filled-out and signed copy of the Declaration form (please submit electronically, email, to address below).
