- Performance Work Statement Request for Quote Attachment 1 - Past Performance WD 2005-2407 Rev 12 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. The solicitation number F3C0BW3141A001 is being issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66. This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 813110 with a size standard not to exceed $7.0 million (U.S.) Protestant Parish Coordinator and Director of Religious Education for 12 months with a base year and two option years. See attached Request for Quotation and Performance Work Statement. Place of delivery is Minot AFB, ND 58705 and is FOB destination. The provision 52.212-1, Instructions to Offerors-Commercial Items, 52.252-1, and 252.209-7999 apply to this acquisition. The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. The government will award a firm-fixed price contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The other factors considered will be the ability to demonstrate the capacity to fulfill this requirement in accordance with the attached performance work statement. Award shall be based on best value to the government. Technical Acceptance Criteria will be evaluated on a pass/fail basis. In order to be technically acceptable the Offeror must: •1. Have successfully completed (1) contracts of similar size and nature within the past (5) years. (Offeror shall complete attachment 1 and submit with offer.) Similar size and nature is defined as: Minimum value of $15,000 annually and be actively engaged as a contractor, either prime or subcontractor, performing Protestant Religious Education services as defined by by the North American Industry Classification System Code (NAICS Code) 813110. The government advises Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Offerors are also reminded to complete Offeror Representations and Certifications on-line at www.sam.gov prior to submitting an offer. Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable: 52.204-10, 52.209-6, 52.219-6, 52.219-28, 52.222-3, 19, 21, 26, 35, 36, 37, 52.223-18, and 52.232-33. Clauses included by reference are: 52.204-7, 9, 52.223-5, 6, 52.225-13, 52.247-34, 52.253-1, 252.204-7003, and 252.204-7004 Alt A. Clauses included by full text are: 52.252-2, 252.203-7000, 252.203-7002, 5352.201-9101, 5352.242-9000, 5352.242-9001, and 5352.223-9001. DFARS Clause 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executives Orders Applicable to Defense Acquisition of Commercial Items applies to this acquisition and the additional DFARS clauses cited in the clause are applicable: 252.225-7001 Buy American Act and Balance of Payments Program, and 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports. Clause 52.222-41, Service Contract Act of 1965 is applicable. There is no Defense Priorities and Allocations System (DPAS) rating applicable for this procurement. All offerors must submit a quote by 2:00 pm Central Standard Time on Thursday, 20 June 2013. Quotes received after that time may not be considered.
