Description
The VA Center for Organization, Leadership and Management Research (COLMR) has a requirement for the administration of patient surveys for two VA Human Subject R&D funded research projects including Identifying mTBI subtypes and their implications for recovery and reintegration and Organizational and Patient Factors Related to Polytrauma/TBI Patient Outcomes. Both projects involve surveys of Veterans with mild traumatic brain injury (TBI) and both will use web and paper modes of administration. Both studies will survey initial samples of 6,000 Veterans (separate samples; not the same 6000 individuals). The Organizational and Patient Factors Related to Polytrauma/TBI Patient Outcomes study will involve a single administration period. The Identifying mTBI subtypes and their implications for recovery and reintegration study will involve two administrations periods, approximately one year apart (Time 1, Time 2). Each administration will feature multiple contacts of non-respondents. Example tasks include: 1.Final formatting/preparation/printing/photocopying of all pre-survey notification letters, study fact sheets and patient surveys. 2.Perform final formatting of the survey instrument to maximize readability and ease of completion, while minimizing paper usage and weight. 3.Preparation of postcards, outgoing and business reply (BRE) envelopes 4.Survey packet preparation with unique identification number on each survey, outgoing mailing label preparation and insertion of surveys and BRE into correct matching outgoing envelope 5.Data entry of address corrections from "Return Service Requested" feature on outgoing survey envelopes. 6.Merge/purge of mailing lists to mail to non-respondents 7.Provision of web survey programming and web hosting of survey for subjects selecting web option. 8.Data entry of incoming paper surveys 9.Data entry verification of incoming paper surveys 10.Provision of data file in Excel format with separate data file for each type of survey (one for paper responses, one for web responses) 11.Provision of final mailing lists for each contact phase of the project 12.Regular status reports of the incoming surveys (exact timing of reports to be determined) via email and telephone. 13.Provision of survey tracking document showing status for each Veteran subject 14.Materials: pre-survey notification letters (single-sided one page letter), pre-survey notification #10 outgoing envelopes, invitation letter (single-sided one page letter), fact sheet (double-sided one page), invitation outgoing envelopes #10, standard-size postcards, survey booklets, cover letter (single-sided one page letter), survey outgoing envelopes 10x13", survey business reply envelopes(BRE) envelopes 9x12" with barcode, reminder letter (single-sided one page letter) and reminder letter outgoing envelopes #10. All are black ink on white paper. The VA seal will be printed in black ink. All outgoing envelopes will have "Return Service Requested" feature. 15.Inclusion of VA-provided Research Brochure with all paper survey packets. The Research Brochure is a tri-fold, double-sided one page brochure measuring 8.5x14", which will be provided by VA. The intention of this notice is to seek sources that could potentially provide these services and to get a better idea of the type of socio economic set-aside which may be used.
