Solicitation Number: VA248-13-I-1310 Notice Type: Sources Sought Synopsis: RFI PREFACE: This is a Sources Sought Notice only. This is not a request for quotes or proposals. No formal solicitation document exists at this time. If suitable potential vendors are found as a result of this RFI, a formal solicitation will be issued at a later date on the Government Point of Entry (GPE) that this RFI was posted. PROJECT: Procure Navilas Laser Photocoagulation System (see "Background" section) BACKGROUND: The Orlando VA Medical Center (Lake Nona) is conducting market research and seeking information from all interested vendors regarding the availability of a commercial item(s) described in this Request for Information (RFI). This requirement is for the procurement of: 001: P/N 100238; Navilas Laser Photocoagulation (Outright Purchase) This equipment is a navigated laser system used in the treatment of diabetic retinopathy and other retino-vascular diseases in both navigated focal and peripheral treatments. Salient Characteristics for this equipment are as follows: Static field of view: 10 degrees; 30 degrees, 50 degrees Dynamic field of view: 110 degrees Color/IR (Mydryatic & Non-Mydriatic) and FA/Red-Free. 002: Operator & Service Maintenance Manual 003: Biomedical Training (if Applicable) 004: Operator Training for all applicable staff No "gray market" items, recycled, or refurbished parts allowed. Delivery location: Orlando VAMC, 13800 Veterans Way, Orlando FL 32824. The NAICS code is 339112. REQUESTED INFORMATION: All questions listed below must be answered completely and submitted with your information. The vendor is encouraged to provide any additional information that it believes would be useful to the VA. 1) Does your business offer the commercial items listed above (see "Background")? 2) Is your business the actual manufacturer of the Navilas Laser Photocoagulation System, or are you an authorized distributor? Authorization must be from the manufacturer. 3) Is your business a United States business? If not, identify the country. 4) Is your business registered in the System for Award Management, aka SAM (https://www.sam.gov/portal/public/SAM/)? 5) Regarding the size of your business, is it more than 100 employees? 6) Is your company any of the following small business socio-economic categories (list all that apply)?
