- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is RFQ No. IHS1196525. The solicitation is issued as request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. (iii) The solicitation document and incorporated provisions and clauses are those in effect Includes amendments thru FAC 2005-66 effective February 28, 2013 (Note: FAR amendments are updated on their effective dates). (iv) The associated NAICS code 335313, which has a small business size standard of 750 Employees, is applicable. A firm fixed-price contract is anticipated to result from award of this solicitation. (v) A list of contract line item number(s) and items, quantities and units of measure, (including option(s), if applicable). Also, see attached performance work statement. 1. The contractor shall provide two brand new automatic transfer switches (ATS). a. Two (2) ASCO Series 7000 Automatic Transfer Switches (400A, 3 Pole, 3 Phase, 277/480V, 60Hz b. One (1) ASCO Series 5400 Power Manager for the above 400A Automatic Transfer Switches. (Brand Name or Equal may be considered. If offering an "equal" product of the brand name manufacturer, it must meet the salient physical, functional, or performance characteristic specified in this solicitation. You must clearly identify the item by the brand name, if any; and make or model number(FAR 52.211-6). 2. The contractor shall provide the technical knowledge, labor, tools, materials, supplies, transportation, equipment, loading, rigging, supervision and complete successful installation of the two automatic transfer switches. (vi) Description of requirements for the items to be acquired. (vii) The contract line item(s) will be delivered 30 days ARO (or as negotiated) to the Indian Health Service, Tohatchi Health Center, 07 Choosgai Drive, Tohatchi, New Mexico 87325. (viii) FAR provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. In accordance with FAR 52.204-7(b)(1) Central Contractor Registration, all prospective awardees shall be registered in the Sam.gov database prior to award, during performance, and through final payment of any contract resulting from this solicitation. (ix) FAR provision at 52.212-2, Evaluation-Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. In addition to price, the quote evaluation will be based on a best value trade off of the following factors: Quality of the product, Technical capabilities, and Past performance, that is approximately equal to price. (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.
