- EEO NON SEGREGATED DAVIS BACON DAVIS BACON BIDDER AFFIRMATION NEW VENDOR FORM W-9 TAX EXEMPT The South Bronx Job Corps Academy is currently soliciting for 0n-site Kitchen Appliance Maintenance. The South Bronx Job Corps Academy is a federally funded residential training facility for youth between the ages of 16 and 24. The program operates year round with 200 out of our 300 trainees living on the premises during their enrollment. The Academy is operated for the United States Department of Labor by ResCare, Inc. ALL CONTRACT ACTIONS ARE SUBJECT TO THE APPROVAL OF THE UNITED STATES DEPARTMENT OF LABOR IN ACCORDANCE WITH FEDERAL REGULATORY PROCEDURES AND MANDATES OF THE FEDERAL REGISTER. THIS IS NOT AN OPPORTUNITY TO CONTRACT WITH THE UNITED STATES DEPARTMENT OF LABOR. The following documents attached hereto will be incorporated herein and made part of this agreement: •· Davis Bacon Act (40 U 27a attachment B) Certification of Non-Segregated Facilities Pursuant to FAR 52.222.21 (attachment C) Equal Opportunity and Affirmative Action Requirement Pursuant to FAR 52.222.26 and FAR 52.222.27 Federal Acquisition Clauses (FAR) *ANY REGULATIONS NOT INDICATED IN THIS AGREEMENT ARE REINENFORCEABLE BY NEW YORK STATE, LOCAL GOVERNMENT LAW AND FEDERAL REGULATION Companies participating in the bid process will submit all bid packages by February 15, 9AM EST. Contractors responding to the solicitation will submit the following documentation with their packages; -CURRENT RESUME -PROFESSIONAL REFERENCES -NEW YORK STATE LICENSES (MUST BE VALID & CERTIFIED TO OPERATE ON SPECIFIED EQUIPMENT) FOR ALL TECHNICIANS WORKING ON-SITE -PROOF OF CURRENT PROFESSIONAL LIABILITY COVERAGE -MSDS SHEETS (WHERE APPLICABLE) -CONFIRM TESTING EQUIPMENT IS UP TO NATIONAL STANDARDS *Packages missing documentation, will be excluded from the bid process. Scope: Provide Maintenance for the following; Located in the Main Kitchen SUNFIRE BURNER RANGE /w DUAL OVEN HOBART MIXER THREE (3) PITCO FRYER TWO (2) SOUTHBEND SALAMANDER TWO (2) VULCAN CONVENTIONAL OVEN UNIVEX SLICER FOUR (4) WELL STEAM TABLE ALL CURRENTLY FUNCTIONING Emergency Response Time within 4 hours preferred (specify non-working hours and calendar days) All work is to be performed between 8AM & 4PM Please Provide Break Down Insurance Policy to specify coverage you are willing to provide for additional services beyond preventative maintenance, if any. Contract Terms: The period for this contract will be for One Year beginning Aril 1, 2013 ending March 31, 2014. Renewal of the contract may be subsequent to annual solicitation of services. Additional work may be requested outside of the said agreement which shall only be authorized in writing by the Executive Director, Administrative Manager or Purchasing Agent upon mutual agreement of both parties.
