- Description
- The Department of Veterans Affairs, Lebanon, Pennsylvania Veterans Administration Medical Center (VAMC) has a requirement to locally fabricate and repair artificial limbs for eligible beneficiaries of the Lebanon Medical Center. The period of performance shall be from the date of award for 12 months with four (4) one-year option periods exercised at the Government's discretion. All work shall be performed in strict accordance with all specifications, statement of work, terms and conditions contained therein. The solicitation shall state all specifications required. The NAICS code for this requirement is 339113. This requirement will be 75% small business set-aside. The Government anticipates making multiple awards from this solicitation. The solicitation is estimated to be released on or about 8 February 2013. ONLY qualified Contractors, with technical expertise and are an established place of business within a max of 100 miles from the city of Lebanon should submit a proposal for consideration. The following VA forms MUST be submitted with offer and can be found at http://www.va.gov/vaforms; VA Form 2130, Inspection Sheet - Prosthetic Dealer and VA Form 2793, Shop Data Sheet (Artificial Limbs). Once solicitation is released, all questions shall be submitted in writing to Contracting Officer, Stacy Ling at stacy.ling@va.gov. Phone calls will not be accepted.
