Description
This is a Combined Synopsis/ SOLICITATION for Commercial Items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisitions Procedures for commercial items found at FAR 13.106(1), as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The Request for Quotation (RFQ) number is N00259-13-T-0076 and it is issued for reference only. The closing date is 12/03/2012 @ 09:00 a.m. Pacific Standard Time. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-61. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses; www.Acquisition.gov. This is a 100% small business set-aside solicitation. Navy Drug Screening Laboratory, Request responses from qualified sources capable of providing Preprinted Labels for use with Specimen Processing. All vendors must submit approximately 100 labels to be tested for 15-30 days to ensure that labels will stand up to the conditions outlined in the attached œSpecification of Labels  Period of Performance: 01 Jan 2013 to 31 Dec 2013 CLIN 0001 QTY: 960 EA $_______________ Period of Performance: 01 Jan 2014 to 31 Dec 2014 CLIN 0001 QTY: 960 EA $_______________ Period of Performance: 01 Jan 2015 to 31 Dec 2015 80 each to be delivered within one week of the month of intended use. 1 EA= 1000 labels See Specification of labels: Transcode S-475 Polyolefin material, s/S-475 permanent acrylic adhesive or equal, special die configuration, 4 label set 1.5  X 1.5  (consisting of 4 smaller labels): (1).292  x 1.5  (2).292  x 1.5  (3).292  x 1.5  (4).625  x 1.5  Barcode symbology Code 128, 11 character number, incrementing number sets, each label unique, finished and separated FAN FOLDED of 100 sets each. Printed barcode characters and label itself are oriented œladder style  (vertically instead of œpicket fence  horizontally). The labels shall consist of 2.3 mil white polyolefin or polyester thermal transfer and conforms to the additional specification below. The release liner substrate shall be perforated to allow each label to be removed individually form the label roll. The die cuts shall be precise and consistent throughout each roll of labels, and the labels shall be 100% usable. The manufacturer ™s label rolling process shall not produce any kinking or creasing of labels. The gap between labels shall be 1/8 inch. Storage stability of rolled labels shall be 12 to 18 months at 70 “ 80 degrees F and 75% relative humidity after receipt of labels. The labels, including release liner will be applied around the circumference of specimen bottles and applied along the horizontal and vertical axis of tubes and vials. The individual labels shall be easily pealed back and removed from the centerline of the release liner. The labels shall not show any evidence of lifting or flagging at the edges after 3 days of exposure at room temperature, exposure to 20 minutes in an autoclave at 250 degrees F and 15 psi (total time of 40 in the autoclave), or after repeated removal and reapplication on test tubes and aluminum cups during chemical extraction procedures. The release liner and in particular the label adhesive shall be flexible enough to allow label position adjustments for minimum of 3 hours following by permanent adhesion within 24 hours on paper, metal, plastic, and glass surfaces. The label shall be quickly removable and transferable between glass tubes and metal recovery cups during the extraction process without leaving adhesive residue on the glass or aluminum and without smudging the barcode to the point of non-readability. The labels may be exposed to chemicals such as methanol, ethyl acetate, methylene chloride, isooctane, iodopropane, isopropanol, 1-chlorobutane, dimethylsulfoxide, triethylamine, iodomethane, tetramethyl ammonium hydroxide, ammonium hydroxide, sodium hydroxide, potassium hydroxide, phosphoric acid, acetic acid, sulfuric acid, and hydrochloric acid. Labels should not fall off, or shrink under such exposure, and the print should not smear to the point of non-readability. This requirement also applies to exposure in a laboratory autoclave. Contractor shall use a black thermal transfer ribbon. The ribbon resin shall provide a stable, dense, abrasion resistant, erasure resistant barcode with excellent edge definition, and the print should not smear to the point of non-readability. There will be no missing or duplicate numbers. The resulting barcodes shall have a print contrast and light area reflectance of 98% or better and 0% reflectance of dark areas. Number scheme will be an identification of San Diego Lab (S), last two digits of calendar year, month indicator, and batch and specimen identifier. Labels will be packaged in accessioning identification of batch/specimen. Both the label print and adhesive shall last for a minimum of 5 years at room temperature on paper documents, and a minimum of 2 years on specimen bottles in freezer storage. The large part and 2 of the small parts of each label will display a barcode and human readable characters. The barcode will duplicate the human readable characters with the exception of the dashes (-) which will be omitted in the barcode. The remaining small part of each label will be human readable only and will be the same information as the other 3 parts of that label. Actual barcode and human readable configuration will be as follows: Human Readable: Position 1: œS  for San Diego Positions 2 & 3: Last two digits of calendar year Position 4: Letter designation for the month: œA = January, œB =February, œC =March, œD =April, œE =May, œF =June, œG =July, œH =August, œJ =September, œK =October, œM =November, œN =December. Position 5: œ0  (Zero) Position 6: œ- œ (Dash) Positions 7, 8, & 9: Batch #. Starting at 001 and running thru and including 800 for each month. Position 10: œ- œ (Dash) Positions 11, 12, & 13: Specimen # within the batch. Starting at 001 and running thru and including 100 for each batch # (Positions 7 thru 9).
