Description
PERFORMANCE WORK STATEMENT GENERAL The Bay Pines VA Healthcare System (BPVAHCS), Bay Pines Florida has a requirement for two (2) on-site medical physicists for the base year, March 1, 2013 through February 28, 2014 with one (1) one-year option. FACILITY The BPVAHCS is located at 10000 Bay Pines Boulevard, Bay Pines, Florida. The local operation at Bay Pines is a unique healthcare system with a 76 year history of serving veterans. The Healthcare System provides a full continuum of veteran centered care and has sustained the highest performance levels among complex VA medical centers while caring for more than 95,000 patients with over 1.2 million outpatient visits annually. Bay Pines VA Healthcare System is situated on 337 acres on beautiful Boca Ciega Bay, adjacent to the Gulf of Mexico, and a short distance from downtown St. Petersburg, Florida. The Bay Pines VA Hospital is a major teaching, tertiary care facility with in-house radiation oncology including a linear accelerator. A daily volume of patients receiving radiation is ~40 patients. SCOPE OF WORK The Contractor shall provide two (2) full-time equivalent (FTE) medical physicists. Pricing shall include all travel, lodging, per diem and any other associated costs. A.Definition of FTE: One full-time equivalent (FTE) is defined as a contractor's employee working eight hours per day, five days per week. A contractor's employee must be present at the VA facility and must be actually performing the required services for the period specified in the contract. The contract cost will be decreased for each hour of absence, or quarter hour thereof, when the number of hours on duty is less than forty (40) hours per week; such adjustment will be reflected on the contractor's invoice for that billing cycle. The payments for any leave, including sick leave or vacation time are the responsibility of the contractor. B.Contractor shall provide medical physicist services to perform the requirements of this contract. The contractor medical physicists work in the Radiation Oncology Service to provide expert knowledge and competence in the field of Radiation Oncology Physics. The contractors shall be available, when necessary, for consultation with the Radiation Oncologist and provide advice or direction to technical staff when radiation treatments are being planned or when veterans are being treated. Where possible, the contractors shall be present to observe and/or help supervise complicated simulations and/or treatment set-ups. Contracted employees shall maintain a high level of knowledge of Radiation Safety for veterans, visitors and staff by attending seminars and reviewing current literature pertaining to the field of Radiation Oncology. The contractors provides advice and consultation, as well as principles, methods, applications and state-of-the-art technology in Radiation Oncology to provide direction and guidance on critical and complex issues C.Contracted employees shall work to ensure a safe work environment, to protect all veterans and staff member from unwarranted exposure to ionizing and non-ionizing radiation, lasers, electrical devices and sealed sources. D.Contracted employees shall obtain all necessary output measurements on each treatment unit for the purposes of daily/weekly/monthly Quality Assurance, accurate treatment & dose delivery. The dosimetry system shall be initially established via initial beam specification and calibration and shall be maintained thereafter through daily, monthly and annual checks and calibrations. All unusual applications of the dosimetry system shall be confirmed by measurement(s) prior to actual clinical use. All repairs of the treatment unit that may impact the dosimetry system shall be reviewed by the contractor prior to returning the unit to clinical use. The contractors shall participate in the specification, selection, and acceptance of radiation-producing machines, accessories, and computerized treatment planning systems. The physics staff should also supervise arrangements for proper maintenance of this equipment. The contractors shall periodically evaluate all equipment for continued utility, appropriateness, reliable performance, age, and condition and make recommendations regarding practical life span, obsolescence, and replacement. E.Contracted employees shall perform quality assurance on all proposed treatment plans prior to commencement of patient therapy to ensure its feasibility, safety and accurate delivery and to document such QA processes in the patient's chart. F.Contracted employees shall load all above stated data into the treatment-planning computer systems and to perform systematic reviews of all the technical aspects of the treatment delivery system to ensure that the treatment prescription is being delivered in a safe and medically effective manner. The contractor shall specify and monitor method(s) to calculate MUs or treatment times and ensure independent review(s) of such calculations. Any individual having appropriate training and experience may perform the initial calculation(s). Independent review of said calculation(s) shall be performed by the contractors within a specified period of time. Prepares written and oral reports, and makes recommendations regarding use of photons and electron beams. G.The contractors are expected to be proficient in clinical treatment planning and shall be expected to perform all portions of radiation treatment planning. The contractors shall develop and maintain a method for the regular (usually weekly) and systematic review of the charts of all veterans under radiation treatment. The contractors shall perform a final chart review at the end of the course of radiation treatment in order to confirm that the prescribed dose has been delivered, and to document the total doses delivered to critical structures. H.The contractors must be competent in the use of computers for testing, dosimetry, data collection, storage, retrieval and analysis. The contractors are directly responsible for all quality assurance of all physics related equipment, computer software, treatment devices and brachytherapy/sealed sources. Contracted employees shall work in collaboration with necessary personnel and provide oversight when necessary to install, maintain, and utilize computer programs that are essential to continually improve the treatment planning process. I.The contactors shall work in conjunction with the radiation safety officer to measure and analyze radiation levels in all areas of the clinic, both interior and the surrounding areas to ensure the levels are within the predetermined levels based on nationally accepted regulations. Analyzes the environmental aspects of radiation and radioactivity, and performs mathematical calculations and other complex and analytical procedures. Performs scientific assessments related to radiation hazards within the Service, and makes recommendations concerning corrective actions for identified weaknesses and/or deficiencies. J.The contractors shall be responsible for the training of dosimetrists and junior physicists. The contractors shall oversee, check and sign off on all the work of dosimetry and junior physicists. Contracted employees shall assist the service chief in their advisory/supervisory control. The contractors are responsible to appropriately respond to emergencies. K.The contractors adhere to the Quality Control Programs for the treatment equipment and devices utilized in their respective testing. Analyzes criteria and adjusts standards in response to program developments. Contracted employees shall provide assistance to engineers and other physicists in avoiding unwarranted radiation exposures. Contracted employees shall manage/update/edit all policies and procedures directly relating to physics/dosimetry, this includes but is not limited to: treatment planning, treatment delivery, emergency procedures for machine malfunction, corrective action for machine malfunction, radiation safety, etc. The contractors shall develop and maintain a quality management program (QMP) for the dosimetry system(s) and all applications pertinent thereto. Said QMP shall define explicit evaluation criteria intended to ensure that the prescribed dose is delivered in a safe, consistent and accurate manner. The radiation oncology physicist(s) shall provide the Medical Director with regular (at least annual) written reports of these activities. Quality management of radiation therapy equipment is primarily an ongoing evaluation of functional performance characteristics. Accordingly, contractors shall develop, implement, supervise, and periodically review all QMP policies and procedures that pertain to radiation therapy equipment. The contractors are responsible for the design, implementation and periodic review of all aspects of the QMP that involve the use of radiotherapy equipment. L.The Chief of Radiation Oncology provides administrative direction with assignments in terms of broadly defined missions or functions. The contractors have the responsibility for independently planning, designing, and carrying out programs and a variety of projects. Results of the work are considered technically authoritative and are normally accepted without significant change. M.The contractors shall work to ensures radiation equipment and treatment units are operating at safe and efficient levels. The contractors shall perform detailed review of treatment documentation to ensure accurate treatment delivery. The contractors, having developed expertise in the Radiation Oncology area, can resolve most conflicts as they arise, coordinating the work with others as necessary. This shall be accomplished by using good judgment in interpreting and adapting policies, regulations, and precedents on own initiative in terms of established objectives. Contracted employees shall assist dosimetrists and junior physicists in difficult treatment plans and complex calculations for veterans' treatments. The practice of radiation oncology often involves the implementation of new procedures and technologies, so the contractor must, in conjunction with the Chief of Radiation Oncology, define basic standards of practice and develop a reasonably prudent course of action to determine the quality and safety of any new procedures prior to implementation thereof. In those cases where the contractor requires assistance, consultation with experienced colleagues is encouraged. When newly published techniques or procedures are being implemented for the first time within the practice/facility, the contractors should undertake a systematic literature review, make appropriate site visits, observe procedures, and participate under the supervision of colleagues who are familiar with the procedure. The QMP associated with any new procedure should be periodically reviewed and updated. N.The contractors shall assist in establishing and maintaining a Radiation Oncology Dosimetric System to include the following: a.Calibration and parameterization of each treatment beam, ensuring all factors required meet the treatment prescription as written by the physician. b.Collecting dose parameters based on direct measurement. All unusual applications of the dosimetry system shall be confirmed prior to the actual clinical use. c.The contractor, prior to returning the unit for clinical use, shall review all repairs of treatment unit. d.The contractor shall test the various imaging devices, i.e., the CT Simulator, using phantoms with known characteristics. e.Ensure all users of the dosimetric system are properly trained and their knowledge base upgraded, as new technologies are available. f.Attend peer review and other conferences such as, new patient conference to offer suggestions to improve veterans' treatment methods. g.Shall develop and monitor a system of independent review for treatment charts for veterans undergoing treatments within a specified time period, including weekly and final checks. Perform a final chart review to ensure that the end of treatment prescribed dose is accurate. h.Supervise and implement dose measurements to ensure accuracy of prescribed doses. i.Assess charts to eliminate possible misadministration and track variances. O.The contractors shall, at a minimum, be responsible for ensuring that the external beam treatment units and simulator are operated in a manner consistent with Facility's Radiation Safety program and State and Federal regulatory agencies. P. The contractors are expected to develop personal contacts, including a wide range of professionals and administrative personnel throughout the facility necessary to achieve above mentioned responsibilities. Q.The work is a combination of office and clinical activities. It includes working with high voltage equipment. It involves walking, standing, bending and carrying heavy equipment. R.Work involves everyday risks and discomforts that require normal safety precautions typically associated with offices. There is a risk of possible exposure to high-energy radiation. Otherwise the facility is well lighted and heated. The treatment units have special air conditioning systems to maintain a cold environment. S.Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, federal regulations, VA statutes and policy, and VHS&RA policy. Protects the data from unauthorized release or from loss, alteration, or unauthorized deletion. T.Provides care and/or services appropriate to the age of the veterans being served. Assesses data reflective of the veteran's status and interpret the information needed to identify each patient's requirements relative to their age-specific needs and to provide care needed as described in the services policies and procedures. SPECIAL CONTRACT REQUIREMENTS The contractor agrees, in accordance with the terms and conditions stated herein to furnish to and at the Veterans Health Administration, hereinafter called the VA, at 10000 Bay Pines Blvd, Bay Pines, FL the services specified and at the prices specified in Schedule of Supplies/Services. 1. SERVICES: a. The services specified herein may be changed by written modification to this contract. The modification will be prepared by the VA Contracting Officer and is NOT valid until the VA Contracting Officer has signed the modification and the Contractor has received a copy for their records. b. Other necessary personnel for the operation of the services contracted for at the VA will be provided at levels mutually agreed upon which are compatible with the safety of the patient and personnel and with quality medical care programming. c. The services to be performed by the Contractor shall be performed within the VA policies and procedures and the regulations of the medical staff of the VA facility. d. The services to be performed by the Contractor shall be under the direction of the Administrative Officer (COR),Chief of Radiation Oncology Service at the VA Medical Center Bay Pines, FL. 2. TERM OF CONTRACT: This contract is effective for one year with 1 one-year option. 3. QUALIFICATIONS: a. Personnel assigned by the Contractor to perform the services covered by this contact shall be certified by the American Board of Radiology or American Board of Medical Physicists in Radiation Oncology Physics, with clinical experience including accelerator commissioning. The qualifications of such personnel shall also be subject to review the VA Chief of Staff and approved by the VA Medical Center Director. The individuals must be a highly qualified, master's or PhD level medical physicists with ABR board certification in medical physics, with extensive radiation oncology clinical experience (7 years minimum). These positions requires a mastery of the knowledge of theories, radiation principles, concepts, and methodologies to constantly improve radiation treatment Contracted employees shall possess knowledge of qualitative and/or quantitative methods for the assessment and improvement of program effectiveness Contracted employees shall possess knowledge and experience in the commissioning & acceptance process of new Linear Accelerators. Contracted employees shall demonstrate their experience in participation/leadership in said commissioning processes of a minimum of 5 different linear accelerators. Contracted employees shall possess clear understanding of radiation room shielding requirements, construction & collection of data for input into the treatment planning computer system. The contractor must be proficient in all current treatment planning techniques, complex dose computations for 2-D, 3-D, IMRT and brachytherapy treatments. Contracted employees shall possess knowledge and experience in implementing and maintaining an Intensity-Modulated Radiation Therapy (IMRT), Arc Therapy (RapidArc) and Image-Guided Radiotherapy (IGRT) programs as well as all associated quality assurance of said programs. Contracted employees shall possess knowledge and experience in implementing and maintaining a Stereotactic Radiotherapy (SRS/SBRT) program. Contracted employees shall possess clear understanding of collection of data for input into the treatment planning computer system. The contractor must be proficient in all current SRS/SBRT treatment planning and immobilization techniques. Contracted employees shall possess knowledge and experience in implementing and maintaining a High Dose Rate (HDR) Brachytherapy program. Contracted employees shall possess clear understanding of all applicable laws and regulations, supervising a source exchange, acceptance and calibration of a new source, periodic quality assurance of the HDR unit, and collection of data for input into the treatment planning computer system. The contractor must be proficient in all current brachytherapy treatment planning techniques and applicators. 4. WORK HOURS: a. The services covered by this contract shall be furnished by the Contractor as defined herein. The contractor is required to provide coverage to the clinic during standard work hours. This includes coverage for vacation time, sick leave or any other approved or emergency absence of the physicist. All services shall be performed at the Bay Pines VA Healthcare System, 10000 Bay Pines Boulevard Bay Pines, Florida, 33744. The contract Physicist must be present at the VA, and must be actually performing the required services for payment. The Contractor shall be paid for actual performance only, not for standing by or being on-call. Payment for any leave, including sick leave or vacation time, and holiday pay is the responsibility of the Contractor. Coverage is defined as on-site physics support during business hours. The Contractor shall not be required, except in emergency cases, to furnish such services on a national holiday or during non-working hours as described below. b. The following terms have the following meanings. (1) Work Hours: 8:00 a.m. to 4:30 p.m., Monday through Friday. (2) National Holidays: The ten holidays observed by the Federal Government, i.e., New Year's Day, Martin Luther King's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans' Day, Thanksgiving, Christmas, and any other day specifically declared by the President of the United States to be a national holiday. (3) Off-duty Hours: Monday through Friday, 4:30 p.m. to 8:00 a.m. weekends, and national holidays. The contract Physicist may occasionally be required to work outside normal scheduled hours (i.e. for equipment calibrations or emergent treatments). These situation will be handled on an individual basis. 5. EDUCATION, TRAINING & TRAVEL: a.The contractor shall cover all expenses for conferences and meetings that pertain to the physicists' ongoing continuing education (CE) to ensure that he/she are competent and versed in all current practices in the medical physics field. Such conferences include, but are not limited to the ASTRO (American Society of Radiation Oncology) annual meeting and the AAPM (American Association of Physicists in Medicine) annual meeting. The contractor shall be responsible for all expenses, both tuition and travel, for such events. The contractor will be paid normal work hours for pre-approved attendance. b.The travel costs for training and education that pertains directly to ensuring safe operations of the clinic's equipment (linear accelerators) will be supported by the VA. The contractor will pay for the physicist(s) travel to attend such sanctioned trainings and will invoice the VA for the expenses once classes have been established. The cost of the travel will be determined in accordance with Government Travel Regulations prior to contracted employee attending the education. A modification will be prepared by the VA Contracting Officer prior to contracted employee attending the training and is NOT valid until the VA Contracting Officer has signed the modification and the Contractor has received a copy for their records. c.The education tuition costs will be paid by the VA directly to the training agency. 6. PERSONNEL POLICY: The contractor shall assume full responsibility for the protection of its personnel furnishing services under this contract. To carry out this responsibility the contractor shall provide worker's compensation, professional liability insurance, health examinations, income tax withholding, and social security payments. Such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. 7. MONITORING PROCEDURES: a. Time and Attendance: (1) All contract personnel shall sign in and out in the Radiation Oncology Service timekeeper's log on a daily basis. (2) Contractor personnel shall carry a cell phone at all times. (3) Emergency service must have prior approval by the Contracting Officer's Representative (COR). b. Contract Services: (1) Machine calibration shall be documented daily, weekly, and monthly as required. (2) Review of treatment plans and patients' charts shall be documented according to the procedures employed by Radiation Oncology Service. (3) Quality assurance (QA) shall be documented and submitted to COR and Chief Radiation Oncology Service according to the procedures employed by Radiation Oncology Service to ensure contract services are being provided. c. Verifying VA Official: The Administrative Officer/Chief of Radiation Oncology Service will serve as the Contracting Officer's Representative (COR) and will ensure compliance with the contract terms. 8. OTHER: No contract will be awarded nor payment made to any individual or entity that appears on the Centers for Medicare & Medicaid Services (CMS) formally Health and Human Services Office of Inspector General's (HSS/OIG) List of Excluded Individuals/Entities. HIPAA Compliance - Health Insurance Portability and Accountability Act of 1996: The successful Contractor shall be required to be in compliance with HIPAA requirements. Before service commences, the Contractor shall provide evidence of professional liability insurance coverage in accordance with contract. 9. PRIVACY ACT: The Contractor's personnel shall agree to comply with the Privacy Act of 1974 (the Act) and the VAHCHS rules and regulations issued under the Act during and after the execution of this contract. Any information about an individual that is maintained by the VA, including, but not limited to, education, financial transactions, medical history, and criminal or employment history or any record that contains the person's name, or the identifying number, symbol, or other identifying particular assigned to the individual, shall be kept CONFIDENTIAL. 10. PRIVACY TRAINING: All Contractors and business associates must receive VHA National Privacy Policy training and Cyber Security Awareness training annually. a.For contractors and business associates who do not have access to VHA computer systems, this requirement is met by receiving VHA National Privacy Policy training, other VHA approved privacy training or contractor furnished training that meets the requirements of the HHS Standards for Privacy of Individually-identifiable Health Information as determined by VHA. Proof of training is required. b.For contractors and business associates who are granted access to a VHA computer system, this requirement is met by receiving VHA National Privacy Policy training or other VHA approved privacy training. Proof of training is required. 11. CONFIDENTIALITY AND SECURITY MEASUREMENTS: a. VA will provide the Contractor with access to pertinent patient medical information for the purposes of providing a service to VA. The Contractor shall ensure the confidentiality of all patient information and shall be held liable in the event of the breach of confidentiality. Any person who knowingly or willingly discloses confidential information from the VA may be subject to a fine. b. Patient Medical Records: Contractor personnel who obtain access to medical records or access hardware/software that may store drug or alcohol abuse data, sickle cell anemia treatment records, records or tests or treatment for or infection with HIV, medical quality assurance records, or any other sensitive information protected under 38 U.S.C. symbol 4132 or symbol 3305, as defined by VA, shall only be given access to the records absolutely necessary to perform required duties. Any contractor-employee required to access pertinent data shall not disclose to other Contractor employees that are not involved in the performance of this task or to any other source. Any violation of these provisions may involve imposition of criminal penalties. c. Patient Medical Records Handling, Storage, and Retention: The Contractor shall store all records in a locked/secured room during the performance period of this requirement. VA will not allow the contractor's staff to work off-site. However, the Contractor's Project Manager is responsible for ensuring that their employees maintain the records in a safe and secure environment. The Contractor shall retain the records in a secured storage area for Period of 60-days after final delivery. The Contractor shall dispose/destroy the records at the Contract's primary place of performance by means of shredding all documents so that none of the data is legible. The Contractor shall retain certificate of destruction records. d. Property in Possession of Subcontractors: In accordance with FAR Part 45.510, the Contractor shall require any of its subcontractors possessing or controlling Government property to adequately care for and maintain that property and assure that it is used only as authorized by the contract. The Contractor's approved property control system shall include procedures necessary for accomplishing this responsibility. e. Confidentiality Statements: Contractor staff shall sign and follow confidentiality statements as required. f. Privacy Act notification: As a result of this task, the Contractor shall be subject to the Privacy Act of 1974. The Contractor agrees to comply with the Privacy Act, 38 USC 5701, and 38 USC 7332, and all future amendments. 1)The Contractor is required to include the Privacy Act notification contained in this contract in every solicitation and resulting subcontract and in every subcontract awarded without a solicitation, and include this clause, including this subparagraph, in all subcontracts awarded under this contract which requires the design, development, or operation of such a system of records. 2)In the event of violations of the Act, a civil action may be brought against the agency involved when the violation concerns the design, development, or operation of a system of records on individuals to accomplish an agency function, and criminal penalties may be imposed upon the officers of employees of the agency when the violation concerns the operation of a system of records on individuals to accomplish an agency function. For purposes of the Act, when the contract is for the operation of a system of records on individuals to accomplish an agency function, the Contractor is considered to be an employee of the agency. 3)"Operation of a system of records," as used in this clause, means performance of any of the activities associated with maintaining the system of records including the collection, use, and dissemination of records. 4)"Record," as used in this clause, means any item, collection, or grouping of information about an individual that is maintained by an agency, including, but not limited to, education, financial transactions, medical history, and criminal or employment history and that contains the person's name, or the identifying number, symbol, or other identifying particular assigned to the individual, such as a fingerprint or voiceprint or a photograph. 5)"System of records on individuals," as used in this clause, means a group of any records under the control of any agency from which information is retrieved by the name of the individual or by some identifying number, symbol or other identifying particular assigned to the individual. g. Contractor personnel performing work under this contract shall satisfy all requirements for appropriate security eligibility in dealing with access to sensitive information and information systems belonging to or being used on behalf of the Department of Veterans Affairs. The Contractor shall be responsible for the actions of those individuals they provide to perform work for the VA under this contract. In the event that damages arise from work performed by contractor provided personnel, under the auspices of this contract, the contractor shall be responsible for all resources necessary to remedy the incident. Printed output containing sensitive VHA data will be stored in a secured area, and disposed of properly by shredding or similar methods. Under the provisions of the Privacy Act of 1974 as amended, personnel performing work under this contract have an obligation to protect VA information indefinitely? Furthermore, it is the contractor's responsibility to notify the Information Management staff when access to Information Management systems is no longer needed by personnel performing work under this contract. h. In performing this agreement, the Contractor shall be considered part of the Department of Veterans Affairs (VA) for purposes of 38 U.S.C. symbol 5701 and 7332. Its employees may have access to patient medical records to the extent necessary to perform this contract. Notwithstanding any other provision of this agreement, the contractor and its employees may disclose patient records and individually-identified patient information, including information and records generated by the Contractor in performance of this agreement, only pursuant to explicit disclosure authority from VA. i. All contractor personnel and any subcontracted employees, if applicable, accessing the VISTA system will be required to sign and abide by all VA security policies, and applicable VA confidentiality statues, 38 U.S.C. symbol 5701, 38 U.S.C. symbol.e 7332, and the Privacy Act, 5 U.S.C. symbol 552a. The VA will provide access applications and security agreements; Contractor shall ensure the confidentiality of all patient information and shall be held liable in the event of the breach of confidentiality. Due to the confidential nature of patient records, all work must be completed in areas that provide reasonable security. All documents are confidential are protected under the Privacy Act of 1974, as amended. All vendor personnel shall be required to observe the requirements imposed on sensitive data by law, federal regulations, VA statutes and policy, DM&S policy and the associated requirements to insure appropriate screening of personnel. j. The database created by the Contractor under this agreement, the adverse drug event reports provided to the Contractor by VA, and documents created from analyzing this database, the adverse drug event reports, and patient medical records are medical quality assurance records protected by 38 U.S.C. symbol 5705, its implementing regulations at 38 U.S.C. records may be disclosed only as authorized by symbol 5705 is a criminal offense under 38 U.S.C. symbol 5705(e). k. The treatment and administrative patient records created by, or provided to, the contractor under this agreement are covered by the VA system of records entitled "Patient Medical Records - VA (24VA136). 12. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: Background Investigations: All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor shall be responsible for the actions of those individuals they provide to perform work for VA. Position Sensitivity - The position sensitivity has been designated as Low Risk. Background Investigation - The level of background investigation commensurate with the required level of access is National Agency Check with Written Inquiries. Contractor Responsibilities The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse VA within 30 days. The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are able to read, write, speak and understand the English language. The contractor shall submit or have their employees submit the following required forms to the VA Office of Security and Law Enforcement within 30 days of receipt: "Standard Form 85P, Questionnaire for Public Trust Positions "Standard Form 85P-S, Supplemental Questionnaire for Selected Positions "FD 258, U.S. Department of Justice Fingerprint Applicant Chart "VA Form 0710, Authority for Release of Information Form "Optional Form 306, Declaration for Federal Employment "Optional Form 612, Optional Application for Federal Employment
