- SOURCES SOUGHT: Payson and Southwest (Buckeye/Goodyear) CBOC The Carl T Hayden VA Medical Center is seeking Contractors to operate and provide Community Based Outpatient Clinic (CBOC) services for the Department of Veterans Affairs in Payson, Arizona and in Buckeye/Goodyear (Southwest), Arizona. The facility shall be physically located within the city limits of Payson, Arizona (Gila County) and Buckeye/Goodyear, Arizona (Maricopa County). Contract personnel shall provide primary care services and mental health services for eligible enrolled VA beneficiaries. Estimated number of VA beneficiaries in Payson is 575 and the estimated number of VA beneficiaries in Buckeye is 345. Historically, the CBOC contract has been priced on a per-member/per-month/capitated basis. Contract period is date of award for one base year with four option year renewals. The NAICS is 621498; size standard is $8.5M. Interested vendors should reply to the Contracting Officer via email with the subject line: CBOC Payson and Southwest, by close of business on November 10, 2012. Business size certification. 5. That the firms CCR registration and ORCA certifications are accurate and current. 6. That the firm or any employee is not currently on the excluded parties listing system at http://epls.arnet.gov/ Your responses will help determine whether the resulting solicitation will be set aside for small business or open to vendors of all sizes. As of now it will be open to all. The anticipated award date is February 2013. Note: This is not a solicitation or request for proposal and no formal solicitation document exists at this time.
