- NOTICE TO CONTRACTOR This is a COMBINED SYNOPSIS/SOLICITATION for commercial items, Solicitation number N00259-12-T-1022 is issued as a notice to procure from only one source IAW FAR 13.106-1(b). This proposed contract action is used for Radiology/Radiation Oncology Department for Service Maintenance. All replacement parts, software, and upgrades must be 100% compatible with the existing proprietary equipment. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is BrainLab Inc. CAGE Code: 6EZB8, NAICS: 511210 Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that BrainLab Inc is the only company that compatible with the existing proprietary equipment and have the proper training and authorization to service BrainLab equipment. The software and component of the radiation therapy systems is under constant improvement by the manufacture. Critical patches, software updates and component updates as required are essential to the continued operation of the system. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The Request for Quotation (RFQ) number is N00259-12-T-1022. The closing date is October 12, 2012 @ 12:00 PM Pacific Daylight Time. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-60. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The following factors are to be used to evaluate offers: Technical Capability and past performance. Provide three (3) references, addressing that you have provided the same or similar items in the last 3 years. Technical capability and past performance, when combined, are significant evaluation factors that will be used to arrive at a best value award decision. Technical capability and past performance are considered more important than price. The proposed contract is restricted to domestic sources under the authority of (FAR 6.302-3.) Foreign sources, except Canadian sources, are not eligible for award. The Government will only consider firm fixed-price quotations. Offeror must complete and submit with the quotation FAR provision 52.212-3 Offeror Representations and Certifications-Commercial Items (May 2008.)
