- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W912DY-12-T-0427 and is issued as an RFQ. The associated North American Industrial Classification System (NAICS) code for this procurement is 335999 with a small business size standard of 500 employees. This requirement is a 100% Small Business set-aside. FOB Destination shall be: Reynolds Army Community Hospital Attn: Facilities Management FT. Sill, OK 73503 Delivery POC: Richard Julian (580) 558-3056 richard.julian@amedd.army.mil Reynolds Army Community Hospital requires the following items, Brand Name or Equal, to the following: Industrial quality Accelerometer w/10' cable. IMI Sensor Model # 607A11 or equivalent. Qty: 256 each Industrial quality Accelerometer w/30' cable IMI Sensor Model 607A11/030BZ or equivalent Qty: 64 each Sensor Mounting Pad IMI Sensor Model080A93 or equivalent Qty: 320 each 12 Channel Switch Box IMI Sensor Model 691B42/691020 or equivalent Qty: 32 each Large Adhesive Kit, 2-Part Epoxy IMI Model 075A06 or equivalent Qty: 32 each 11" x 11" AHU Inspection Doors Qty: 32 each All quotes must be sent to Paul Daugherty at Paul.C.Daugherty@usace.army.mil and Christopher.M.Andrews@usace.army.mil. Quotes are required to be received no later than 12:00 Noon Central Time, Saturday, September 29, 2012. If the quoter will provide the brand-name item, it shall state that in the quote. If an equivalent item will be provided, then the quoter shall submit the manufacturer's pdf format (adobe acrobat) cut-sheet or specification sheet with the quote.
