- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Reference number W912P5-12-T-0058 is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. The requirement is unrestricted, and the associated NAICS code is 327320 and size standard is 500 personnel. Clauses may be accessed electronically in full text through http://farsite.hill.af.mil The following provisions apply: FAR 52.212-1 Instructions to Offerors-Commercial, FAR 52.212-3, Offerors Representation and Certification Commercial Item, FAR 52.212-4 Contract Terms and Condition-Commercial Items, FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, and DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statues or Executive Orders Applicable to Defense Acquisitions of Commercial Items. The date and time quotes are due is 1:00 PM, 15-Aug-2012, to Ray.Kendrick@us.army.mil or fax to Attn: Ray Kendrick, 615-736-7124. It is the responsibility of the offeror to submit your quote by the date and time mentioned above. Offeror shall submit quote on CLIN 0001 as noted below. CLIN 0001 Concrete for Obey River Campground 1. Scope of Work: The Contractor shall furnish and deliver 5,000 psi concrete, and fiber added. Concrete shall be per cubic yard price that includes the haul rate to Obey River Campground at 100 Obey River Park Road, Monroe, Tennessee 38573. The contractor will haul and place each load of concrete exactly on locations as directed by an onsite government representative. 2. Delivery: An average of (8) eight trucks shall be provided for delivery each day to complete the work order. Quantities listed are approximate quantities for each area and are subject to change and not guaranteed. The contractor will provide weight tickets to a government representative for each area and will maintain a running total of all concrete delivered. The delivery of concrete will not be accepted during wet weather conditions and deliveries are subject to be cancelled by the government at the beginning of each work. 3. Concrete Specifications: The contractor shall provide approximately 360 cubic yards (quantities are estimated and are not guaranteed) of concrete. Product: 5000 PSI Concrete Quantity: 360 Unit of Measure: Cubic Yards Price:$ _______________ 4. The Contractor shall furnish concrete as scheduled by the government representative on or before 30 March 2013. The delivery shall be limited to Monday through Friday during daylight hours unless prior approval has been obtained from the Resource Manager. QUOTATION INSTRUCTIONS: 1. Past performance information: To establish its responsibility the offeror shall submit a statement regarding previous experience in performing comparable work, business and technical organization, and financial resources,. The requested statement submitted shall have the following: a minimum of one (1) up-to-date banking reference for their firm and a minimum of three (3) past performance references for similar work. a. The banking reference should include the financial institution's legal name, complete physical address, a point-of-contact (POC) at the financial institution, the POC's telephone number, fax number, and email address.
