- Combined synopsis soliciation package for Bulk Oxygen, medical gases and cynlinder rentals CLA-12-R-0040 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number CLA-12-R-0040 is for a Contractor to provide bulk oxygen, medical gases and cylinder rentals in accordance with the Indefinite Delivery/Indefinite Quantity (IDIQ) contract awarded to the Claremore Indian Hospital The responses will be evaluated based upon the proposed pricing for the base year and four (4) pre-priced option years product pricing. The attached solicitation is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55. This acquisition is full and open competition. The NAIC code is 325120 and the small business size standard is 1000 employees. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The clause found at FAR 52.212-4, Contract Terms and Conditions - Commercial Items applies to this acquisition. The clause found at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. FAR 52.212-1 Instructions to Offerors - Commercial applies to this acquisition. Offers are due September 7, 2012, 4:30 p.m. CST at Claremore Indian Hospital, Support Services, 101 South Moore Avenue, Claremore, Oklahoma 74017.
