- Pricing Spreadsheet - Updated 7/13/2012 Site Visit - Notes, Questions, and Answers. Updated 7/13/2012 Statement of Work - Updated 7/13/2013 UPDATED JULY 13, 2012: The purpose of this amendment is to make the following changes and updates: 1) Notes, questions, and answers taken from the site visit are attached. 2) Update the "Statement of Work" attachment to reflect correct tank sizes and performance periods. 3) Update the "Pricing Spreadsheet" attachment to reflect a) correct tank sizes and b) change the liquid argon pricing factors from "Cost Per Tank" to "Cost Per Liter", and "Estimate Per Year" in tanks to "Estimate Per Year" in liters. 4) All quotes shall be submitted to Michelle Creenan by email at michelle.creenan@usuhs.edu. Confirmation of quotes received will be provided. Quotes received by any other method will not be accepted. _____________________________________________ UPDATED JULY 9, 2012: The purpose of this amendment is to make the following changes and updates: 1) Site Visit (optional) details are attached. Registration is required. The due date is COB, Tuesday, July 10, 2012. 2) Update the "Pricing Spreadsheet" attachment to reflect the new performance periods and the correct tank size for liquid argon. 3) Update the "Questions and Answers" attachment. ______________________________________________ UPDATED JULY 6, 2012: The purpose of this amendment is make the following changes: 1) The correct size of the argon cylinders is 180 liters per tank. 2) A site visit for this solicitation will be scheduled and any interested offeror may attend. Details of the site visit (time, place, etc.) will be posted on Monday, July 9. 3) The period of performance has been changed to: Base Year: 11 AUG 2012 - 10 AUG 2013 Option Year I: 11 AUG 2012 - 10 AUG 2013 Option Year II: 11 AUG 2012 - 10 AUG 2013 4) The due date for all quotes has been changed from Monday, July 9, 2012 to Friday, July 20, 2012 by 5:00 PM (EST). Any questions are due on Monday, July 16, 2012 by 12:00 PM (EST). _________________________________________________________ SOLICITATION (Original): The Uniformed Services University of the Health Sciences (USUHS) is a medical university and a Department of Defense Agency. USUHS has a requirement for the delivery of various liquid gases. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) issued in accordance with FAR Part 12-Acquisition of Commercial Items and FAR Part 13-Simplified Acquisition Procedures. It is the responsibility of the contractor to monitor FedBizOpps (http://www.fbo.gov) for the issuance of any amendments to the solicitation. This solicitation incorporates all relevant provisions and clauses in effect through the Federal Acquisition Circular (FAC) 05-58. An option year contract (base plus two option years) is anticipated for award. This procurement is set aside for small businesses only. The applicable NAICS code for this requirement is 242690 - Other Chemical and Allied Products Merchant Wholesalers. The business size standard is 100 employees. To be eligible for award, the contractor MUST be registered in the Central Contractor Registration (CCR) website (https://www.bpn.gov/CCRSearch/Search.aspx) and the Online Representations and Certifications Application (ORCA) (https://orca.bpn.gov/) by the close date of this solicitation. Quotes received from offerors not registered in CCR and ORCA may not be considered. The current liquid gas contract is held by Cryo Express, LLC under purchase order HU0001-09-M-BV18. The total contract amount for HU0001-09-M-BV18 (base plus two option years) is $78,235.54. The period of performance (base year) of the new contract is expected to begin on July 11, 2012 for twelve (12) months. For additional details, please see Statement of Work and Provisions and Clauses (attached). Contractors shall submit quotes utilizing the Excel spreadsheet attached. The government intends to award a single firm-fixed priced (FFP) purchase order to the offeror representing the lowest price technically acceptable. The government reserves the right to award without discussions. USUHS is part of the Department of Defense and is tax exempt. A copy of our tax exempt certificate will be provided to the selected offeror. Do not include taxes in your quoted price. Additionally, interested offerors shall provide the DUNS (Dun & Bradstreet) with the quote. Any questions regarding the solicitation must be received by 9:00 AM (EST) on Thursday, July 5, 2012. Questions must be emailed to clarinda.washington@usuhs.edu. Responses to questions will be addressed as an amendment to the solicitation posted on FedBizOpps. No questions will be answered over the phone. Quotes must be received by 10:00 AM (EST) on Monday, July 9, 2012. Quotes shall be accepted by email only. No other method of submission will be accepted. Email all quotes to clarinda.washington@usuhs.edu. Late quotes will not be considered.
