- Description
- The US Army Medical Command Southeast Region has a requirement for Ossatron System Shockwave Therapy (approximately 25 patients). The Government intends to award a sole source contract for this requirement pursuant to 10 U.S.C. 2304(c)(1) and FAR 6.302-1 only one responsible source and no other supplies or services will satisfy agency requirements. The suggested contractor is: Premier Shockwave, Inc., 750 Hammond Drive, Building: 18, Suite: 200, Atlanta, Georgia 30328-5532 Ossatron System Extracorporeal Shock Wave Technology system (must meet FDA approval) mobile unit with certified technician for the treatment of chronic plantar fasciitis (heel pain). Please provide per each patient pricing for unilateral treatment of chronic plantar fasciitis and bilateral treatment of chronic plantar fasciitis. Pricing is to include all cost to cover but not limited to mobile unit, technician, and mileage. This is not a request for proposal and no solicitation will be issued. However, companies that can provide an equal or like product must submit their technical capabilities to include technical specifications of their product, past performance information, price and delivery time within three (3) calendar days from the date of publication of this notice. This information will be reviewed by the Government technical representatives from Eisenhower Army Medical Center for consideration for future requirements. This procurement will be prepared in accordance with FAR Part 13, using Simplified Acquisition Procedures. Point of Contact: Linda M.
