- STATEMENT OF WORK CONSTRUCTION SERVICES FOR PROJECT 620A4-09-110 Renovations to Dental Clinic at Castle Point Background This project will renovate the currently unoccupied space at VA Castle Point for the dental service. The goal of this project is to optimize suite layout in order to comply with fire code requirements, improve suite accessibility under the Americans with Disabilities Act (ADA), and layout office space to provide for proper patient monitoring and privacy. Objectives The objective of this project is to renovate unused and asbestos contaminated space for the dental service currently as detailed in the project drawings and specifications. Period of Performance The period of performance is 450 days from the notice to proceed. Place of Performance / Place of Delivery The place of performance is VA Castle Point. Scopeor Tasks (for services) The scope of the project is to provide all materials, equipment, supervision, and personnel necessary to accomplish the work outlined in the attached drawings and specifications. The Contractor will be responsible to coordinate and schedule work to minimize to short duration and shall be expected to plan work typically during duty hours of 8 AM to 4:30 PM eastern time except as noted in the general conditions and as approved by the Contracting Officer (CO). The project includes demolition and renovation of electrical, architectural, masonry, plumbing, security, and ventilation systems. The C&A requirements do not apply, and the Security Accreditation Package is not required. Government-Furnished Property, Material, Equipment, or Information (GFP, GFM, GFE, or GFI) There is no GFP, GFM, GFE, or GFI under this contract. The Contractor is required to relocate dental equipment items on the weekend prior to activation by the clinic. Security The contractor will be required to comply with campus safety and security requirements as outlined in the General Requirements specification. In general, this will require: 1.The contractor must obtain campus identification for all workers 2.The contractor will be limited to only VA furnished contractor keys for the work area. Access to any other areas necessary for work will require escort by the Contracting Officer ™s Technical Resource (COTR). Safety Codes / Certification / Licensing The contractor is required to obey all safety codes as identified in the attached specifications and drawings. All construction personnel on site are required to obtain an Occupational Safety and Health Administration (OSHA) 10 hour construction certification, and the site competent person for the contractor must obtain an OSHA 30 hour construction certification. The contractor will be required to comply with federal and state code and certification requirements pertaining to lead paint and asbestos as outlined in the attached bid package. Travel No travel requirements are anticipated in the project. Special Material Requirements Material requirements are as specified in the attached drawings and specifications. Any Other Requirements unique to the solicitation. Contractor shall meet, review and comply to the Hazardous Evaluation Form (SOP 138-022) Section A and C and other SOP(s) of the facility The project is rated a Categorical Exclusion under NEPA. The C&A requirements do not apply, and that a Security Accreditation Package is not required.
