Description
AF Form 996 HazMat Form Wage Determination Statement of Work WAWF Price Schedule Well # 6 Solicitation Solicitation FA4620-12-Q-A056 is issued as a written Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55 (effective 27 Apr 2012). This procurement is under the North American Industry Classification System Code (NAICS) 811310. The small business size standard is $7,000,000.00. This procurement is issued on an unrestricted basis. Unit Small Business Specialist is Michael P. Gilbert (509-247-4880); link to USAF Small Business, http://www.selltoairforce.org; link to SBA http://sba.gov. The contractor shall remove the pump and motor from Fort Wright Well #6 and visually inspect motor and pump for damage and provide repairs or replacement as directed by the government. Upon award completion, payment will be made through Wide Area Workflow (Attachment Two). The following FAR provisions apply to this acquisition: 52.252-1 - Solicitation Provisions Incorporated by Reference (Feb 1998) This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provisions may include blocks that must be completed by the offeror and submitted with its quotation or offer. In lieu of submitting the full text of those provisions, the offeror may identify the provision by paragraph identifier and provide the appropriate information with its quotation or offer. Also, the full text of a solicitation provision may be accessed electronically at this address: http://farsite.hill.af.mil (End of Provision) FAR 52.212-1 - Instructions to Offerors - Commercial Items (Jun 2008) Addendum to FAR Clause 52.212-1: To ensure timely and equitable evaluation of the quote the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation. Offerors must be currently registered in Central Contract Registry (CCR) to be eligible for award. Quotes shall include the following five (5) items: 1. Pricing Schedules: (See Attachment One). Offeror shall complete all required fields in the Pricing Schedule. Pricing shall be based on F.O.B. destination. 2. Completed Representations and Certifications: Completed Representations and Certifications: as required in FAR 52.212-3 with Alt 1 (accessible at http://farsite.hill.af.mil) and DFARS 252-212-7000. Registration in the online representations and certifications program at https://orca.bpn.gov meets this requirement. 3. Commercial Warranty (See Attachment One) 4. Past Performance: Each offeror shall describe and provide references for three (3) recent product installations of similar complexity to this project. Recent is defined as within the last three years. 5. Signed amendments issued against this solicitation. Quote Submission: Quotes are due no later than 15 May 2012 by 1:00 PM PDT. Quotes may be faxed to 509-247-8685 Attn A1C Christopher Dills, in accordance with FAR 52.214-31, Facsimile Bids (Dec 1989), or E-Mailed to christopher.dills@us.af.mil AND edward.campbell.2@us.af.mil, respectively. Quotes may also be mailed to 110 W. Ent St. Suite 200, Fairchild AFB, WA 99011-8568. Potential offerors are cautioned that there are frequent delays of one half hour or more between sending an email and receipt at Fairchild.
