Locations CLIN structure Attached is the solicitation for the HaiVision warranty. Performance Work Statement This is a solicitation for commercial services prepared in accordance with FAR Subpart 5.207, and as supplemented with additional information included in this solicitation. The solicitation document, incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-58, effective 18 April 2012. All responsible sources may submit a quotation which shall be considered. Prospective offerors must be CCR registered and must complete the ORCA on-line representation or provide a copy of their representation and certification as required by FAR Clause 52.212-3. 1. The following support services are required. HaiVision maintenance only support include software maintenance for firmware, software patches and updates as well as an Advanced Maintenance/Replacement and Support Program with 0800 - 1700 local time coverage for each base. Support is required to sustain the enterprise ACC IPTV system, which supports 12 ACC bases. Costs for repairs and travel will not be separately billed and should be included in the cost of the maintenance support. 2. Period of Performance: BASE: 1 July 2012 - 30 June 2013 OPYR1:1 July 2013 - 30 June 2014 OPYR2:1 July 2014 - 30 June 2015 3. Delivery: FOB Destination Deliver To: ACC/A6OI 180 Benedict Ave Suite 210 Langley AFB, VA 23665 4. EVALUATION: The requirement will be evaluated based on low price technically acceptable (LPTA). 5. Commercial Warranty applies: Please identify any additional warranty information 6. GSA or Open Market: Please indicate if your quote is open market or on a GSA Schedule (provide specific schedule) 7. This requirement is 100% Small Business Set-Aside. 8. The Question period for this solictiation will be from 27 April 2012 to 11 May 2012, Noon Eastern Standard time. All answers will be posted on FBO.
