Description
Responses to Questions The purpose of modification/amendment #1 is to change the period of performance dates, extend the quote due date and to answer questions that were submitted in response to the RFQ (see changes below and attached responses to the questions). _________________________________________________________ The Uniformed Services University of the Health Sciences (USUHS) and the Armed Forces Radiobiology Research Institute (AFRRI), located on the Walter Reed National Military Medical Center, in Bethesda, MD have a requirement for various detergents, disinfectants and related supplies for use on AFRRI's Lynx Model 542LXi Rack Washer, Lynx Model 542Lxi Tunnel Washer, and animal laboratory facility. It is anticipated that an award will be made for a base period of one (1) year, with four (4) one (1) year options. This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 - Acquisition of Commercial Items and FAR Part 13 - Simplified Acquisition Procedures. It is the responsibility of the contractor to monitor FedBizOpps (http://www.fbo.gov) for the issuance of any amendments to the solicitation. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-56, effective April 2, 2012. See attached provisions and clauses. This requirement is set-aside for small businesses under NAICS code is 325611 (Soap and Other Detergent Manufacturing), with a size standard of 750 employees. In order to be considered for award, offerors must be registered in the Central Contractor Registration (CCR) database (www.ccr.gov) and the Online Representations and Certifications Application (ORCA) database (www.orca.bpn.gov) by the close date of the solicitation. Quotes received from offerors not registered in both databases will not be considered. We are currently using Quip Laboratory's bulk (130 gallon) storage tanks and chemical distribution systems to our tunnel and rack washers. Our neutralization system checks all cage-washer detergents for the proper pH before being dispensed to the drainage system. All equipment (bulk storage tanks, pumps, dispensing systems, etc.) to hold and dispense detergents into washers must be provided by the vendor. All equipment (5 gallon storage tanks, dispensing systems, etc.) to hold and dispense the disinfectant must also be provided by the vendor. A company representative must visit the facility at least quarterly to inspect and maintain the equipment to ensure proper functioning and submit a quality control report. Award will be made on a best value determination considering price and non-price factors in accordance with FAR 52.212-2 - Evaluation -- Commercial Items Clause. Evaluation Factors (Listed in order of importance): 1. Price - Offerors must provide a price for each CLIN listed above. The resultant award will be a single FFP award; however for price analysis purposes the offeror must provide the following pricing information for CLIN 0007 (Installation/Training): • Number of labor hours/labor category • Number of individuals/labor category 2. Technical - Offerors must provide product literature, which demonstrates the capabilities of the quoted item(s). At a minimum the product literature must demonstrate that the quoted item meets the minimum specifications outlined above and must include a description of the operating procedures. Offerors must also provide an installation/training plan. The plan, at a minimum, must include the following: • How many days after receipt of order (ARO) work can commence. • How many days installation of the new dispensing system will take? • What is included in on-site training (# of hrs, training material)? 3. Past Performance - The Government shall evaluate the offeror's present and past performance on similar tasks within the past three years. Offerors must provide two (2) customer references for similar work performed. The Government reserves the right to select which reference(s) to contact. The following must be provided for each listed reference: a. Name of client and address b. Name of current point of contact including telephone number, e-mail address and title c. Contract number d. Contract value e. Contract period of performance f. A clear description of the supplies/services provided USUHS may request additional information at any time. Offerors will have an opportunity to address any adverse past performance history. Offerors who do not have any past performance history will not be rated favorably or unfavorably, but will instead receive a Neutral/Unknown rating for past performance. Offerors must clearly state in their quote if they do not have at least two (2) past performance references. Failure to provide this information may render an offeror's quote non-responsive and may not be considered for award. A strong history of relevant past performance may be more advantageous to the Government than a less relevant record of favorable performance. The Government reserves the right to use data provided by the offeror and data provided by alternative sources in conducting its assessment of past performance history. A site visit is scheduled for April 12, 2012 (Thursday) at 1:00 PM EST. Offerors interested in attending the site visit must email the contract specialist (Jody Bester - jody.bester@usuhs.edu) by COB Tuesday. USUHS and AFRRI are located on a secure installation and names must be added to the base access list. Offerors interested in attending the site visit are highly encouraged to use public transportation, as parking is limited. Every effort will be made to secure visitor parking, if needed. Directions to the installation and to USUHS from the main gate will be provided to attendees providing a valid email address. USUHS and AFRRI are part of the Department of Defense and are tax exempt. A copy of our tax exempt certificate will be provided to the selected offeror. Do not include taxes in your quoted price. USUHS reserves the right to award without discussions. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
