- This is a Sources Sought Notice (Request for Information). The Government intends to establish a sole source Blanket Purchase Agreement (BPA) for brand name and exclusively patented surgical instruments and implants used for foot and ankle surgeries for the Department of Surgery at Tripler Army Medical Center, Hawaii to Wright Medical Technology, Inc. The items are as follows: Dartfire Screw Set, Darco Modular Forefoot Set, Darco Modular Rearfoot Set, Charlotte 4/3/3.0/7.0 Multi Use Compression Screw, Charlotte Lisfranc Reconstruction System, Charlotte Carolina Jones Fracture System. Other brands do not provide the same surgical results as the Wright Medical products. Calls will be placed against the BPA as supplies are required. Period of performance will be for five years. NAICS code is 339113. Interested parties shall reply to Pacific Regional Contracting Office VIA EMAIL to Annette.Pickering@us.army.mil. TELEPHONE REQUESTS WILL NOT BE HONORED. Please indicate your interest and/or ability to provide the attached list of items and provide a current copy of your price list for the items NO LATER THAN 3 MAY 2012, 8:00 A.M. Hawaii Standard Time or 2:00 PM Eastern Time. Contractor MUST BE ABLE TO PROVIDE 100% of the items on the attached to be considered for this BPA.
