- ***MODIFICATION ONE(1)*** Modification One(1) is hereby issued to RFQ 12-239-SOL-00030 to incorporate the following changes: Response date is changed to April 26, 2012. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. The current solicitation number is 11-239-SOL-00030 and is issued as a Request for Quotation (RFQ). This acquisition is being conducted under Simplified Acquisitions Procedures in accordance with FAR part 13. The clauses and provisions referenced in this solicitation may be obtained in full text form at www.arnet.gov/far. The NAICS code is 484110, and the small business size standard is 25.5 mil. This is a total small business set aside. The Department of Health and Human Services/Indian Health Service has a requirement for Water Softener salt delivery services located at the Red Lake Indian Hospital in Red Lake Mn. The Government contemplates award of a firm fixed-price, contract resulting from this solicitation. Base + 4 option years anticipated See attachment for additional information.
