THIS IS A COMBINED SYNOPSIS/SOLICATION prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this solicitation. This announcement constitutes the only publication; quotes are being requested and a written solicitation will not be issued. All responsible sources may submit a response, which if timely received must be considered by the agency. The solicitation number M00264-12-T-0121, and modifications 0001 & 0002 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50 and DFAR Change Notice 20110506. This proposed acquisition is a 100 percent Small Business Set Aside procurement. The North American Industrial Classification System (NAICS) code for this requirement is 611430. This solicitation is for Diversity Training Services. This contract will be awarded to the offeror that provides the best value to the Government. 1. Diversity Trainiing - MPE Branch Staff Quantity Note: One(1) Unit of training equals NTE eight (8) hours of instruction. Item Name: Diversity Training Course - MPE Branch Staff/Instruction time NTE 24 hours Total Purchase Request price shall include ODC/Travel. 3 EA 2. Diversity Training- Equal Opportunity Advisors Quantity Note: One (1) Unit of training euals NTE eight (8) hours of instruction. Item Name: Diversity Training Course - Equal Opportunity Advisors (EOAs) / Instruction Time: NTE 8 HOURS / Total Purchase Request Price shall include ODC/Travel. 3 EA 3. Diversity Training- Diversity Review Board Quantity Note: One (1) Unit of training euals NTE four (4) hours of executive-level instruction. Item name: Diversity Training Course - Diversity Review board (DRB) / Instruction Time: NTE 4 Hours/Total Purchase Request Price shall include ODC/Travel. 1 EA 4. Diversity Training- Executive Steering Committee Quantity Note: One (1) Unit of training euals NTE four (4) hours of executive-level instruction. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision. The Offeror is required to provide their Federal Tax ID Number, Contractors' DUNS Number and Commercial and Government Entity (CAGE) code with their quote. The CAGE code entered must be for the name and address provided by the Contractor.
